靶重建放大扫描对肺孤立性结节病变的诊断意义

摘要 目的 :探讨靶重建放大扫描技术对肺孤立性结节病变的诊断价值。方法 :对 63例患者先行常规CT平扫 ,选定结节处为兴趣区 ,行薄层靶重建放大扫描 ,层厚、层距为 2mm ,FOV为 160。结果 :42例恶性病变中有 3 7例表现为深分叶 ,占88 1% ;2 1例良性病变中有 15例表现为浅分叶 ,占 71.4%。恶性结节中 15例内部出现条状低密度支气管征 ;7例出现血管集中征 ;8例出现空泡征 ;病变内部出现液化坏死 13例 ,其中恶性病变 11例。 2 2例出现钙化 ,包括 12例良性病变和 10例恶性病变。 2例错构瘤内均见小面积脂肪性低密度影。结论 :靶重建放大扫描比普通CT扫描可提供更多的信息 ,对良恶性病变的鉴别诊断有一定价值。恶性病变大多数为深分叶 。 Objective:To explore the diagnosis value of the target reconstructing and magnifying scan for pulmonary solitary nodules. Methods: After conventional CT scan, nodules were selected as the region of interest (ROI) and the target reconstructing and magnifying scan with 2 mm thick and 2 mm space were performed in 63 patients with pulmonary solitary nodules, FOV was 160. Results: Of the 42 patients with malignant disease, 37 displayed deep lobulations in nodular margin (88.1%); Of the 21 patients with benign disease, 15 demonstrated shallow lobulations in nodular margin (71.4%). Among the malignant nodules, 15 patients displayed strip low-density bronchial sign. Seven patients had vascular focus sign, 8 had vacuolar sign. Of the 13 patients with liquefaction necrosis, 11 had malignant disease. Twenty-two patients had calcification, including 12 patients with benign and 10 with malignant diseases. Small fat low density was seen in two hamartomas during the scans. Conclusion: The target reconstructing and magnifying scan can provide more information than that of the conventional CT scan. It has some diagnostic values in differentiating benign and malignant diseases. Most of the malignant diseases are in deep lobulations, and most of the benign ones are shallow lobulations or without lobulations.
