摘要
目的探讨老年2型糖尿病患者肺功能10年变化规律。方法入选研究对象共57例。Ⅰ组为20例不吸烟健康老年人;Ⅱ组为37例不吸烟2型糖尿病(DM)患者。肺功能按常规方法检查并进行10年随访。结果10年前、后肺功能值组内相比,DM组肺活量(VC)、用力肺活量(FVC)、第1秒用力呼气量(FEV1)、呼气峰流速(PEF)、最大通气量(MVV)改变均明显下降(t值分别为4.90,3.38,5.33,4.49,4.75,P均<0.01),对照组10年前、后肺功能无明显改变。对照组与DM组比较,初始值VC(t=6.40),FVC(t=4.63),FEV1(t=5.97),PEF(t=3.55),MVV(t=5.49)及10年后测量值VC(t=7.05),FVC(t=5.63),FEV1(t=7.22),PEF(t=6.10),MVV(t=6.04)统计学差异显著(P<0.01)。DM组的肺功能各项指标的年改变值与不吸烟健康老年人组比较无明显差别(P>0.05)。10年间DM组的一氧化碳弥散量(DLCO)明显下降(t=10.58,P<0.01),10年前后DM组的DLCO值均较正常值明显降低(t值分别为-12.69,-23.02,P<0.01)。结论老年2型DM患者肺功能随增龄而减退;老年2型DM患者较健康老年人,肺通气功能明显降低;老年2型DM患者与不吸烟健康老年人10年间年改变值统计学无明显差异。DM组的DLCO较正常值明显降低。
Aim To study the changes of pulmonary function in elderly patients with type 2 diabetes mellitus(DM) in 10 years. Methods Fifty-seven elder non-smoke rs were selected, including 37 patients with type 2 diabetes mellitus with an av erage age of (68±4) years, weighing (75.1±2.8) kg (DM group), and 20 age, heig ht and weight-matched healthy controls (health group).The routine lung function tests were followed up for over 10 years.Results During the 10 year follow up, the vital capacity(VC),forced vital capacity (FVC),forced expiratory volume in f irst second (FEV1),peak expiratory flow (PEF),peak expiratory flow (MVV) decreas ed significantly(t=4.90, 3.38, 5.33, 4.49, 4.75, P< 0.01) in DM group. There wer e no changes of pulmonary function in the control group before and after follow up. Compared with health elder group,the initial and 10 year follow-up values of VC,FVC,FEV1,PEF,MVV were significantly different (t=6.40, 4.63, 5.97, 3.55, 5 .49, P< 0.01; t=7.05, 5.63, 7.22, 6.10, 6.04, P< 0.01). The change of pulmonary function in DM group after 10 years was not different from the health group (P > 0.05). The diffusing capacity of carbon monoxide (DLCO) in DM group was signific antly decreased (t=10.58, P< 0.01) in 10 years, significantly lower than the nor mal DLCO. (t=-12.69, -23.02,P< 0.01). Conclusion The pulmonary function of DM is decreased with age in the senile. The pulmonary function of DM patients is wo rse than that of health elder. DM patients show insignificantly statistical diff erence from health elders in the change of pulmonary function in 10 years, with the DLCO decreased significantly compared with the normal DLCO.
