摘要 目的 探讨严重的急性呼吸综合征 (SARS)的影像学特征。方法 回顾临床诊断的 5 0例 SARS的影像学检查方法与诊断价值。结果  5 0例均行胸片 (后前位和右侧位或左侧位 )检查 ,其中 12例患者多次摄床边胸片 ,11例患者行 HRCT检查 ,未行 MRI检查。5 0例胸片均发现阴影 ,双侧肺内片状密度增高影 2 7例 (5 4%)和 /或磨玻璃样影 2 2例 (44 %) ,一侧肺内片状密度增高影 19例 (38%) ,双侧肺内间质改变呈网状阴影 4例 ;病变短期内呈游走性和进展 ,吸收慢并且恢复后部分病例肺内纤维化 ;1例经胸部透视对肺内和胸膜病变进行了鉴别 ;11例经HRCT检查进一步了解肺内和纵隔病变。结论 凡临床诊断或疑似 SARS病例短期内应进行影像学检查 ,并进行短期拍胸片动态观察。 Objective To study the examination methods and image characteristics of Severe Acute Respiratory Syndrome(SARS).Methods To analyze the image examination methods and image characteristics of 50 cases with SARS. Results All patients were checked by chest X-ray films(including posterior-anterior, right or left lateral positions).12 were photographied by bed-side chest X-ray film and 11 cases by HRCT scans. All films showed abnormal shadows,27 with patch high indensity shadows in bilateral lung fields(51.9%);22(44%) with ground-glass pacecification.19 cases(38%) with patch in increased indensity in single lung fields,4 (8%) with net-like interstital changes in bilateral lung fields. The shadows were changing rapidly with no definite locations. The absorb of the shadows was slow and in some cases with pulmonary fibrosis. Conclusion For suspected SARS cases, The chest X-ray examinations are the first chosen method. HRCT is the next choice,if necessary. The SARS can be confirmed when patch shadows, ground-glass opecification, or net-like interstital shadows appeared in lateral or bilateral lung field with locations changing quickly.The diagnosis is supported by clinical and epidemic history.
