摘要 目的 检测膀胱肿瘤组织、尿脱落细胞和外周血中的端粒酶逆转录酶 (hTERT)和c mycmRNA表达 ,探讨二者的临床意义及关系。方法 采用RT PCR法同时检测 2 8例膀胱移行细胞癌 (TCCB)患者和 15例对照的组织、尿脱落细胞和血细胞中的hTERT和c mycmRNA表达。结果 肿瘤组织hTERT和c myc的mRNA阳性表达率分别为 10 0 % (2 8/2 8)和 6 7.8% (19/2 8) ,对照组为 0 (0 /15 )和 2 6 .7% (4 /15 ) ;两指标在尿脱落细胞中的阳性率分别为 85 .7% (2 4 /2 8)和 71.4 % (2 0 /2 8) ,对照组为 13.3%(2 /15 )和 2 6 .7% (4 /15 ) ;血细胞中为 78.6 % (2 2 /2 8)和 5 7.1% (16 /2 8) ,对照组为 6 .7% (1/15 )和 33.3% (5 /15 ) ;肿瘤组与对照组比较具有显著性差异 (P <0 .0 1) ,两指标不同组间比较具有相关性 (P <0 .0 1)。结论 hTERT和c mycmRNA在TCCB中的表达具有相关性 ,在TCCB不同标本中表达具有一致性 ,可作为膀胱肿瘤的良好诊断指标 ,hTERT较c myc优 ; Objective To investigate the expression,correlation and clinical values of human telomerase reverse transcriptase (hTERT)and c myc mRNA of tumor tissues,urine exfoliative and blood cells in transitional cell carcinoma of the bladder (TCCB).Methods Using reverse transcription polymerase chain reaction(RT PCR)assay, we simultaneously examined hTERT and c myc mRNA in 28 TCCB patients' tumor tissues, blood samples, urine exfoliative cells and 15 control samples.Results hTERT and c myc mRNA were detected in 28 of 28(100%) and 19 of 28(67.8%) in tumor tissues, 24 of 28(85.7%) and 20 of 28(71.4%) respectively in TCCB urine samples.In blood samples, the two items were detected 22 of 28(78.6%) and 16 of 28(57.1%), while the expression positive rate of the two items were none (0/15) and 4 of 15 (26.7%), 2 of 15 (13.3%) and 4 of 15 (26.7%), 1 of 15 (6.7%) and 5 of 15 (33.3%) in respective controls.There was significant difference between the positive rate of hTERT and c myc in tumors and controls ( P < 0.01),also correlation between expression of hTERT and c myc mRNA in bladder tumors.Conclusion The expression of hTERT and c myc mRNA was related and consisted in different TCC samples. hTERT was more sensitive and specific than c myc when diagnosing TCCB. Both can be used as diagnostic markers in TCCB, especially from urine exfoliative cells.
