输尿管肾镜气压弹道碎石术治疗复杂性肾结石

Pneumatic lithotripsy for treatment of complicated kidney calculi

目的 观察输尿管肾镜气压弹道碎石术在治疗复杂性肾结石中的疗效。方法 总结分析应用经皮肾穿刺造瘘输尿管肾镜气压弹道碎石术治疗复杂性肾结石 14例的临床资料。结果 13例复杂性肾结石治疗一期碎石成功 ,1例因积脓延期碎石 ,一期碎石成功率达 92 .9%。术后一月结石排净率达 71.4%。结论 经皮肾穿刺造瘘输尿管肾镜气压弹道碎石术治疗复杂性肾结石具有安全 ,高效 ,微创 ,并发症少 ,疗效确切等优点。经皮肾穿刺造瘘准确至结石所在部位是碎石成功的关键 ,术后均应放置内引流支架管。

Objective To evaluate the therapeutic efficacy in patients with complicated kidney calcule who were treated with Percutaneous Nephroscope Lithotripsy(PCNL).Methods From December 2001 to February 2003,14 patients with complicated kidney calcule were treated with PCNL.Results 13 cases could been treated by PCNL at once (92.9%).One case were delayed to treat with Lithotripsy because of pyohydroneprosis.The total stone-free rate is 71.4%(10/14) after 30 days who be cured with PCNL.Conclusion Treatment of complicated kidney calcule with PCNL has been safe,effective,less invasive,less complicate and can avoid open surgery.It is pointed out that the cruxes of successful lithotripsy were the Percutaneous Nephrotithotomy.The ureteral tents shold be placed regularly after operation.
