摘要
目的 :分析脊髓型颈椎病 (CSM)中退变颈椎间盘局部炎性细胞因子的变化 ,探讨 CSM中颈椎间盘退变的生物学机制。方法 :收集 2 5例手术治疗的 CSM患者的退变椎间盘组织 ,取 5例正常椎间盘组织作为对照 ,匀浆后取上清液 ,EL ISA法检测炎性细胞因子 IL- 1β、IL- 6、IL- 8、TNF- α的含量 ;随机取 8例 CSM患者的外周血作检测。 结果 :CSM退变椎间盘组织中炎性细胞因子的含量明显高于自身血清 (P<0 .0 5 ,P<0 .0 1) ,也显著高于正常对照组 (P<0 .0 5 ,P<0 .0 1)。结论 :局部慢性炎症参与颈椎间盘退变过程 ,IL - 1β、IL - 6、IL - 8、TNF-α水平的增高 ,使局部组织中血管内皮细胞受损 ,引起颈椎骨代谢异常 ;这可能是导致颈椎退变逐渐加重的生物学机制之一。
Objective:To investigate the biological mechanisms of cervical spine degeneration in cervical spondylotic myelopathy (CSM) by analyzing the alterations of local inflammatory cytokine levels. Methods: Twenty five degenerative cervical vertebral disc tissues and 8 blood samples were collected from CSM cases;five normal tissues of vertebral discs were taken as control. The contents of TNF α, IL 1β, IL 6 and IL 8 were detected by ELISA. Results:The contents of TNF α, IL 1β, IL 6 and IL 8 were significantly higher in degenerated CSM vertebral disc tissues than that of the normal control and CSM serum. Conclusion:The increased levels of local inflammatory cytokines disturb the bone matrix metabolism by influencing the local vessel function with chronic inflammatory reaction. The above findings may be one of the biological mechanisms of cervical vertebral disc degeneration and deterioration of cervical spine biomechanics in CSM.
