摘要
目的 探讨过敏性紫癜 (HSP)患儿外周血白细胞表面黏附分子CD11b、CD5 4在发病中的作用及其与病情的关系。方法 采用全血直接免疫荧光流式细胞术检测 2 4例HSP患儿及 2 0例正常小儿外周血白细胞表面黏附分子 (AMS)CD11b、CD5 4的表达。结果 (1)HSP组与对照组比较 :淋巴细胞 (L)、单核细胞 (M )、多形核细胞 (PMN)表面CD11b、CD5 4的阳性细胞百分率 (PPC)和 (或 )平均荧光强度 (MFI)普遍上调 (P <0 .0 5或P <0 .0 1) ,其中以M的AMS上调最为显著 ;(2 )HSP肾炎组与HSP非肾炎组比较 :三类白细胞表面CD11b、CD5 4的PPC和 (或 )MFI表达上调 ,CD5 4表达显著上调 (P <0 .0 5或P <0 .0 1)。结论 (1)AMS在HSP的发病中发挥了重要作用 ;(2 )在三类白细胞表面CD5 4表达均同时上调的水平有可能作为判断HSP病情程度的免疫学指标 ;(3)本研究结果可能为HSP的抗黏附疗法提供了一些依据。
Objectives To explore the expression of AMS including CD11b、CD54 on peripheral blood leukocyte surface (PBLS) in children with Henock-Scholein Purpura(HSP).Methods The whole blood immunofluorescence flow cytometry method was used to detect the expression of CD11b、CD54 on surface of lymphocytes(L)、monocytes(M) and polymorphnuclear leukocytes(PMN) in 24 children with HSP(16 without nephritis and 8 with nephritis) and 20 control children.Results (1)PPC and /or MFI of CD11b、CD54 on PBLS in children with HSP were universally higher than those in control group, and AMS on M were the most outstanding (P<0.05 or P<0.01). (2) There were higher PPC and /or MFI of CD11b、 CD54 on PBLS in HSP with nephrit is group were higher than those in HSP without nephritis group , PPC and MFI of CD54 on three kinds of leukocytes were significantly increased (P<0.05 or P<0.01).Conclusions (1)CD11b、CD54 on PBLS might play an importantrole in HSP.(2) The higher expression of CD54 on PBLS might be an immmunologic index to reflect the condition of HSP.
