3D-SPGR冠状位动态增强对肝占位性病变的诊断

摘要 目的:探讨MR三维扰相梯度回波序列(3dimension-spoiledgradientrecalledecho,3D-SPGR)冠状位快速动态增强对肝脏占位性病变的诊断价值。材料与方法:对26例B超、CT未明确诊断的不同大小肝占位性病变病例进行常规单次激励快速自旋回波(SS-FSE)定位、轴位自旋回波(SE)T1WI及快速自旋回波(FSE)T2WI平扫后,用3D-SPGR序列冠状位对病灶定位,行冠状位3D-SPGR序列快速动态增强扫描,分别描出病灶和正常肝实质时间-信号强度曲线,分析其强化特征并与临床、手术病理结果对照。结果:本组肝脏不同占位性病变的肿块,3D-SPGR冠状位动态增强后的强化方式、时间-信号强度曲线不同。结论:3D-SPGR冠状位动态增强对肝脏不同占位性病变具有重要的定性诊断及鉴别诊断价值。 Objective:To evaluate the diagnostic value of 3D-SPGR coronal dynamic contrast-enhanced MRI in liver space occupying lesions.Materials and Methods:Before administration of Gd-DTPA,conventional coronal SS-FSE,axial SE T 1 -weighted,FSE T 2 -weighted and coronal3D-SPGR plain scan images were obtained,then coronal3D-SPGR dynamic con-trast-enhanced MRI was performed immediately,and contrast-enhanced SE T 1 -weighted images were obtained in axial plane af-ter the study.Then time -signal intensity graphs of the lesions and normal liver tissues were drawn.Analysis of the enhancement patterns,and correlate with the clinical materials,operation and pathology results were carried out.Results:The enhancement patterns and time -signal intensity graphs were different among different liver lesions shown by3D-SPGR coronal dynamic con-trast-enhanced MRI.Conclusion:3D-SPGR coronal dynamic contrast-enhanced MRI is important in qualitative and differential diagnosis of various liver lesions.
