摘要
目的 探讨血清胆红素浓度变化对脂质过氧化物及脑梗死的影响。方法 检测 5 8例脑梗死 (CI)患者血清胆红素 (BIL)、氧化修饰低密度脂蛋白 (ox- L DL)、丙二醛 (MDA)浓度 ,并与 32例健康者进行比较。结果 脑梗死组血清总胆红素 (TBIL)、间接胆红素 (IBIL)和直接胆红素 (DBIL)较正常对照组显著降低 (P<0 .0 1,P<0 .0 5 ) ,而 ox- L DL、MDA较正常对照组显著增高 (P<0 .0 1)。直线相关分析显示 :IBIL与 MDA、ox- L DL呈显著负相关 (r1 =- 0 .32 94 ,P<0 .0 5 ;r2 =- 0 .390 5 ,P<0 .0 1)。结论 血清胆红素浓度降低可导致 ox- L DL增多 ,通过氧化损伤、内源性 CO减少等机制 ,促进动脉粥样硬化 (As)、血栓形成和脑梗死的发生发展。低血清胆红素可能是脑梗死的一个新的危险因素。
Objective To explore the change of serum bilirubin concentration and its influence on lipid peroxides and cerebral infarction.Methods Serum bilirubin, plasma ox LDL and MDA were measured in 58 cases with cerebral infarction, and were compared with those of 32 normal controls.Results The serum total bilirubin (TBIL),indirect bilirubin(IBIL)and direct bilirun(DBIL) in CI group were less than those of the controls significantly (P< 0.01 ,P< 0.05 ), while the plasma ox LDL and MDA in CI group were significantly higher than those of normal group (P< 0.01) . The analysis of linear correlation showed that serum indirect bilirubin was negatively correlated with plasma MDA and ox LDL (r 1= 0.3294,P<0.05;r 2 = 0.3905,P<0.01).Conclusions It was indicated that low serum bilirubin concentration could raise the level of plasma ox LDL. Through oxidative stress and reducing internal carbon monoxide, it promote atherosclerosis, thrombosis and cerebral infarction.Low serum bilirubin concentration could be a new risky factor for cerebral infarction.
