179例多发伤的临床分析

Clinical analysis of 179 cases of mulitiple trauma patients

目的分析多发伤的主要病死原因,探讨早期创伤评分及综合治疗的意义。方法对收治的179例多发伤病人进行早期创伤严重度评分(ISS评分)后按分值分组,同时按有无合并颅脑损伤分组,采用包括手术、机械通气、血液净化、胃肠营养等综合治疗。结果ISS评分≥25时多发伤的病死率显著升高,合并颅脑损伤的多发伤患者的病死率明显高于无颅脑损伤的多发伤患者。结论早期ISS评分对伤情评估及预后判断有一定的意义,综合治疗特别是重视颅脑损伤的治疗是多发伤救治成功的关键。

Objective:To analyse major death causes of multiple trauma and investigate the meaning of early injury serious score and synthesis treatment. Methods:179 cases of Mulitiple trauma patients were divided into two groups by calculating each patient's ISS and accompanying brain injury. Such as operation、machinery ventilation、blood purify were used to cure multiple trauma patients. Results:The highest fatlity lies in the mulitiple trauma patients with the severe brain injury. The higher the injury serious scores were. The higher fatlities would be. Conclusion:Early ISS has meaning at gauaging degree of illness and judging prognosis. Synthesis treatment is key to cure the mulitiple trauma patients successfully.
