摘要
[目的]探讨影响青年子宫内膜癌的预后因素。[方法]对52例40岁以下子宫内膜癌患者的临床资料进行回顾性分析。[结果]总5年生存率78.8%。在单因素分析中,妊娠次数、分期、病理类型及分级、肌层浸润、脉管瘤栓、卵巢转移、盆腔淋巴结转移和p53过度表达与预后有关。多因素分析显示仅肌层浸润,脉管瘤栓,盆腔淋巴结转移与预后有关。[结论]肌层浸润,脉管瘤栓,盆腔淋巴结转移是青年子宫内膜癌的独立预后因素。应慎行保留卵巢的手术。Ⅳ期青年患者应争取行手术为主的综合治疗。
To investigate the prognostic factors of survival in patients with endometrial carcinoma40years old or younger.Fifty-two cases of this age group in January1978to June1998treated surgically were retrospectively reviewed.The over all5-year survival rate was78.8%.Univariate analysis showed that infertility,stage,histological type and grade,depth of myometrial invasion,vascular invasion,ovarian metastasis,pelvic lymph nodes metastasis and p53overexpress were related to prognostic factors.While only depth of myometrial invasion,vascular invasion and lymph node metastasis were independent prognostic factors by multivariate analysis.[Conclusion]Depth of myometrial invasion,vascular invasion and lymph node metastasis are independent prognostic factors in younger patients with endometrial carcinoma.Ovarian conservation surgery should be performed with caution.The patients in stageⅣcould be treated with cytoreductive surgery combined with comprehensive treatment.
