围产期窒息新生儿血浆血栓素和前列环素水平的变化及其意义

摘要 【目的】 观察围产期窒息新生儿血浆血栓素和前列环素水平的变化及其意义。 【方法】 将 2 2例围产期窒息新生儿及 2 0例健康新生儿生后 6h内血浆进行放射免疫测定分析。 【结果】 血浆TXB2 (pg/ml)和TXB2 / 6 Keto PGF1α水平 :围产期窒息组明显高于对照组 (P <0 .0 0 1) ,窒息后HIE组明显高于窒息后无HIE组 ,二者之间及与对照组间差异均有显著性 (P <0 .0 0 1) ,6 Keto PGF1α水平各组间差异无显著性 (P >0 .0 5 )。 【结论】 ①窒息新生儿血浆TXA2 和PGI2 发生明显变化 ,此改变与窒息及其程度有关 ,并可能参与了缺血缺氧性脑病的发病过程和发病进程。②血浆TXB2 ( pg/ml)和TXB2 / 6 Keto PGF1α水平在围产期窒息中的作用及其临床意义值得进一步研究 。 To investigate the level of TXB 2 and TXB 2/6 Keto PGF 1α and their significance in neonates with perinatal asphyxia. Blood samples were collected from neonates without perinatal asphyxia and with perinatal asphyxia. 42 neonates was divided into 2 groups: control group( n =20) and perinatal asphyxia group( n =22), the later was divided into 2 subgroups: without HIE ( n =13) and with HIE ( n =9), in 6h after birth, plasma levels of TXB 2, 6 Keto PGF 1α and TXB 2/6 Keto PGF 1α were measured by radioimmunoassay. The level of TXB 2 and TXB 2/6 Keto PGF 1α were significantly higher in neonates with perinatal asphyxia than in neonates without perinatal asphyxia.They were (883.28±422.41) pg/ml vs (202.42±53.91) pg/ml,(5.09±2.01) pg/ml vs (1.85±0.55) pg/ml ( P <0.001);the same results were found among HIE secondary to asphyxia, no HIE, and control group, they were (1 319.38±288.74) pg/ml,(581.36±123.61) pg/ml vs (202.42±53.92) pg/ml;(6.78±1.99) pg/ml,(3.92±0.93) pg/ml vs (1.85±0.55) pg/ml ( P <0.001); the level of 6 Keto PGF 1α was not increased significant among groups. [Conclusions] TXB 2 and 6 Keto PGF 1α plays an important role in the pathogenesis of perinatal asphyxia ,even in the pathogenesis of asphyxia secondary to HIE. Correlations are found between HIE and perinatal asphyxia. The experiment provides a theoretical basis for the modification of asphyxia with drug therapy.
