急相蛋白、IL-6、IL-8早期诊断极低体重儿细菌性感染的价值

EVALUATE THE ROLE OF ACUTE PHASE PROTEINS, BL-6, BL-8 IN EARLY DIAGNOSIS OF VLBWI BACTERIA INFECTION

摘要 为评价急相蛋白、白细胞介素6(IL-6)、白细胞介素8(IL-8)对极低出生体重儿(VLBWI)细菌感染早期诊断价值,对1998年12月~2000年4月收治的42例VLBWI,按细菌感染的诊断标准分感染组23例(8例败血症、15例可疑败血症)和无感染组19例,用散射比浊法及双抗体夹心ELLSA法,于入院当天、第3天、第5天分别检测血清中C反应蛋白(CRP)、结合珠蛋白(HP)及IL-6、IL-8水平。检测结果的比较显示:对VLBWI细菌感染诊断敏感度IL-6为86.96%,CRP为73.91%,IL-8为69.56%。血清IL-6水平与CRP和HP呈正相关,r=0.425,P<0.05和r=0.545,P<0.01。结果提示血甭IL-6有较高敏感性,而CRP敏感性稍低但特异性高(89.47%),且两者有紧密内在联系。因此,故血清IL-6和CRP水平可作为VLBWI细菌感染早期诊断首选指标之一。 To evaluate the role of acute phase protein, interleukin 6(IL-6) , interleukin 8(IL-8) in early diagnosis of very low birth weight infant (VLBWI) bacteria infection, from Dec. 1998 to Apr. 2000, 42 cases of VLBWI had been treated. According to diagnosis standard of infection, they were divided into bacteria infection group (8 cases of septicemia and 15 cases having septicemia risk) and non-infection group( 19 cases). The level of C reactive protein(CRP), haptoglobin(HP), IL-6 and IL-8 in serum were detected by nephelometry and ELLSA, on the first day, the third day and the fifth day during staying in hospital respectively. Results showed: the comparison of the detect shows the difiierent sensitivity in diagnosing VLBWI bacteria infection: 86.9% for IL-6, 73.91% for CRP, 69.56% for IL-8. The level of IL-6 in serum was closely related to CRP and HP: r = 0.425, P < 0.05, and r = 0.545, P < 0.01. Thesefove, IL-6 in serum has a highly sensitivity and CRP has less sensitivity but highly specificity(89.47% ). There was closely related between IL-6 and CRP. Thus IL-6 and CRP in serum can be both used as the preferred indexed in early diagnosis of VLBWI bacteria infection.
