超声乳化技术在外伤性白内障联合手术中的应用

Application of phacoemulsification in unite surgery with trauma cataract

目的 探讨超声乳化白内障摘除技术在外伤性白内障一期联合手术中的应用。方法 回顾性地统计分析、比较我院从1998年以来采用现代白内障囊外摘除联合人工晶体植入(Non-phacoemulsification,N组)和超声乳化白内障摘除联合人工晶体植入(Phacoemulsification,P组)的二种方法治疗111例(111眼)外伤性白内障的疗效。结果 视力:出院最好矫正视力≥0.5,P组有18眼(34.6％),N组有8眼(13.56％),有显著性差异(x2=6.83,0.005>P>0.010);其中视力≥0.8,P组有10眼(22％),N组有4眼(7.4％),有显著性差异(x2=4.437,0.025>P>0.050);外伤性瞳孔散大与欠圆,P组有8眼(15.38％),N组有26眼(44％),有极显著性差异(x2=84.84,P>0.005)。术后低视力的主要因素为角膜散光、人工晶体计算的差异,黄斑水肿,儿童患者还有弱视等因素。结论超声乳化白内障摘除一期联合手术组在出院视力、维持圆形瞳孔、防止虹膜粘连、维持人工晶体正位、清除晶体皮质减轻前房炎症反应等方面具有明显的优势;特别是由于治疗期的缩短、疗效的提高更减轻了患者的痛苦和经济负担,使患者更容易接受治疗。因此,在有条件的医院,对外伤白内障应提倡采用超声乳化白内障摘除联合手术。

Objective To study the application of phacoemulsification in unite surgery with trauma cataract. Methods The review sate analysis of curative effect on 111 eyes trauma cataract with phacoemulsification (P group) and non-phacoemulsification (N group) from 1998. Results There were statistical difference between the two groups in best corrected visual acuity, trauma mydriasis and iris conglutination et al. The mostly complication of low visual acuity alter surgery are astigmatism. Conclusions The therapeutic effects of the phacoemulsification are belter than non - phacoemulsification in unite surgery with trauma cataract.
