摘要
目的:研究糖尿病患者及健康人F波,探讨其对糖尿病周围神经病(DPN)的诊断价值。方法:检测75例糖尿病患者和43例健康人上肢正中神经和下肢胫神经F波,检测参数包括F波最短潜伏期(Flmi)、潜伏期离散度(Fchd)、F波传导速度(Fwcv)、F比值(F ratio)。结果:(1)糖尿病组Flmi、Fchd、Fwcv、F ratio与健康对照组的差别均有显著性意义。(2)糖尿病组F波各参数异常率为18.6%~45.3%、总阳性率为62.7%。(3)糖尿病患者的发病年龄、空腹血糖(FBG)、糖化血红蛋白(GHBA_1C)与F波异常率无显著相关性,病程对F波异常率有显著性影响。结论:F波是诊断糖尿病周围神经病的敏感指标,并可发现亚临床病变,多个参数相结合有助于提高阳性检出率。
Objective: To evaluate the diagnostic value of F wave in diabetic peripheral neuropathy (DPN) by investigating F wave between patient with diabetes mellitus and healthy subjects. Methods: F wave of the median and tibial nerves were measured in 75 patients with diabetes mellitus and 43 healthy persons, F latency minimum (Flmi), F chronodispersion (Fchd), F wave conduction velocity (Fwve) and F ratio were analyzed . Results: ( 1) In the patients with DM, Flmi , Fchd , Fwcv , F ratio were significantly different with control group. (2) The abnormal rate of all F wave parameters were 18.6% - 45.3% , the whole abnormal rate was 62.7% . (3) The duration of DM has significant relation with the abnormal rates of F wave. Conclusion: F wave parameters are sensitive indexes in the diagnosis of DPN, and may detect subclinical DPN. Considering all F wave parameters can improve diagnotic sensitivity.
