一侧股动脉插管双侧髂内动脉介入治疗妇科恶性肿瘤的临床疗效(英文)

Efficacy of the bilateral internal iliac arterial interventional treatment through unilateral femoral artery catheterization in treatment of patients with gynecologic malignant tumor

目的 :通过对晚期或复发性妇科恶性肿瘤介入治疗的疗效分析 ,评价该方法的临床应用价值。方法 :对 4 0例妇科恶性肿瘤病人 (子宫颈癌 2 2例 ,子宫体癌 6例 ,卵巢癌 9例 ,平滑肌肉瘤 1例 ,恶性葡萄胎 2例 )采用一侧股动脉插管双侧髂内动脉灌注化疗药物并栓塞肿瘤血管治疗。每例病人插管 1～ 4次 ,总共插管 98次。在髂内动脉灌注 3～ 4种化疗药物。结果 :按照世界卫生组织对肿瘤治疗制定的统一标准判断疗效 :完全缓解 15例 ,部分缓解 12例 ,无变化 9例 ,病变进展 4例。总有效率 (CR +PR)为 6 7.5 %。所有病人均获得不同程度的近期疗效。结论 :介入治疗既可以术前缩小肿瘤 ,以利于手术切除 ,又可作为综合治疗方法之一和延长病人的寿命 ,其方法简单、安全、有效、副作用小 ,是治疗妇科恶性肿瘤一种比较理想的非手术方法。

AIM: To evaluate the clinical value of interventional treatment for the patients with late or recurrent gynecologic malignant tumors. METHODS: This study dealt with forty patients with uterine cervix cancer (22 cases), uterine body cancer (6 cases), ovary cancer (9 cases), leiomyosarcoma (1 case), and malignant vesicular mole (2 cases). Unilateral femoral artery catheterization, bilateral internal iliac artery infusion chemotherapy and embolization were performed in those patients. Each patient was catheterized for one to four times, totally for 98 times, and three to four kinds of anti-cancer drugs were infused through iliac artery. Then the efficacy was evaluated. RESULTS: According to the standard made by World Health Organization, 15 patients showed complete response (CR), 12 patients partial response (PR), 9 patients no change (NC), and 4 patients progressive disease (PD). The overall response rate (CR+PR) was 67.5 %. CONCLUSION: The approach can be used both to reduce the size of the tumors in pre-operation, and to prolong the patient lives as one of the comprehensive treatments. It is a simple, safe, effective way with less side-effects and an ideal non-operative treatment for gynecologic malignant tumors.
