摘要
目的 探讨侵犯左心房的中心型Ⅲb期肺癌扩大切除术的临床治疗效果。方法 总结 4 0例侵犯肺静脉基底部心房汇合处的中心型Ⅲb期肺癌 ,其中行手术扩大切除 18例 ,非手术治疗 2 2例。结果 手术组比对照组 1、3、5年生存率高 ,统计学上具有显著性差异 (P <0 .0 5 )。结论 全肺或肺叶合并部分左心房切除 ,左心房切除范围不超过 30 % ,术后加强防治肺水肿和健侧肺感染 ,手术安全可靠 。
Objective\ To observe effect of the expand resection of central type lung cancer (Ⅲb )with left atrium invasion.Methods\ 40 cases with central type lung cancer (Ⅲb ) with left atrium invasions were retrospectively analysed,of which,18 cases underwent the expand resections. Results\ The survival rate of the resected group was significantly higher than that of the control group(P<0.05). Conclusion\ Total pneumonectomyect or lobectomyect plus partial left atrium resection was a safe and effective way to extend survival duration of the patients of central type lung cancer (Ⅲb )with left atrium invasions.
