摘要
目的 探讨慢性阻塞性肺病 (慢阻肺 )患者血浆中白三烯C4水平与肺血流动力学关系。方法 采用右心漂浮导管测定了 2 0例慢阻肺患者血流动力学参数 ,并用放射免疫分析法测定了血浆白三烯C4水平 ,同时检测了患者肺功能、氧饱和度。结果 慢阻肺患者静脉血浆白三烯C4水平高于正常人血浆白三烯C4水平 ,肺动脉高压者高于非肺动脉高压者 ,相关分析表明 ,慢阻肺患者血浆白三烯C4水平与肺动脉平均压、肺血管阻力成正相关 ,与心输出量成负相关 ,与氧饱和度成负相关。
Objective To explore the relationship between plasma Leukotriene C 4(LTC 4) level and pulmonary hemodynamics in patients with chronic obstructive pulmonary disease(COPD). Methods The pulmonary hemodynamics parameters of 20 COPD patients were obtained with right heart catheterization.At the same time,the plasma level of LTC 4 was measured with radio immunoassay.Results The plasma LTC 4 level of the systemic vein in normal person was(1.24±0.56)pg/ml,which is significantly lower than of the COPD(1.43±0.12)pg/ml( P <0.01);The plasma level of 12 patients with pulmonary artery hypertension(PAH) secondary to COPD were significantly higher than those of eight patients without PAH( P <0.05).Linear regression analysis shows that plasma LTC 4 level positively correlate with PAPm( r =0.82, P <0.01)and PVR (r = 0.68, P <0.01),but negatively correlate cardiac output(CO)( r =-0.72, P <0.01)and oxygen saturation(SAT)( r =-0.82, P <0.01).Conclusions The results suggest that LTC 4 may directly partcipate in the development of PAH caused by COPD.
