腹腔镜与开腹手术治疗异位妊娠的对照分析

Controlled analysis of ectopic pregnancy treated by laparoscope and intraabdominal operation

目的探讨腹腔镜下异位妊娠的手术方法、适应症、术后疗效及临床应用价值。方法将确诊为异位妊娠有手术指征,采用腹腔镜手术的患者100例为观察组,回顾性分析采用开腹手术的异位妊娠100例为对照组,比较两组的手术情况和术后情况。结果观察组手术时间(40.37±12.86)分钟,对照组手术时间(40.50±12.38)分钟,P>0.05,无差异性;观察组术中出血量为(15.14±5.27)ml,对照组术中出血量为(48.70±4.87)ml,P<0.01,具有显著差异性;观察组术后离床时间为(1.28±0.90)天,对照组术后离床时间为(2.82±1.25)天,P<0.01具有显著差异性:观察组术后2天体温>38℃6例(占6%),对照组28例(占28%),P<0.01,具有显著差异性;观察组平均住院天数(3.42±1.00)天,对照组为(6.74±1.21)天,P<0.01,具有显著差异性。结论腹腔镜手术创伤小,恢复快,术中出血少,住院时间短,是治疗异位妊娠首选的手术方式。

Objective To approach the laparoscopic operation on the therapy of ectopic pregnancy, indication,postoperative effectiveness and clinical practice value. Methods 100 cases of established estopic pregnancy with laparoscopic ope ration as study group,retrospectively analysized 100 cases of ectopic pregnancy with intraabdominal operation as control group,compare the operative situation a nd postoperative situation between the two groups. Results The operation time wa s (40.37±12.86)min in the study group and (40.50±12.38)min in the control grou p, P>0.05,there was no statistically significant difference; The operation bleed ing magnitude was (15.14±5.27)ml in the study group and (48.70±4.87)ml in the control group,P<0.01,the difference was statistically significant;The time off b ed after operation was (1.28±0.90)d in the study group and (2.82±1.25)d in the control group, P<0.01,showed that statistically significant difference;There wa s 6 cases of the patient's body temperature >38%two days after operation in the study group(6%),and 28 cases of that in the control group(28%), P<0.01,there was statistically significant difference between the two groups;The two groups h ad statistically significant difference in the hospitalization days ,that was (3 .42±1.00)d in the study group and (6.74±1.2)d in the control group, P< 0.01.Co nclusions Laparoscopic operation causes little trauma,makes speedy recovery rest itution,redues operative bleeding magnitude and hospitalization days. It is tile prior operative therapy of ectopic pregnancy.
