幽门螺杆菌CagA相关胃炎与IL-8、IL-6及TNF-α关系的探讨

THE RELATION BETWEEN HELICOBACTER PYLORI CagA ON GASTRITIS AND IL-8,IL-6,TNF-α IN SERIUM

目的 :探讨幽门螺杆菌 Cag A相关胃炎与血清 IL- 8、IL- 6及 TNF- α的关系。方法 :将 1 2 0例病人随机分为两组 ,健康对照(AC)组 4 8例 ,慢性胃炎 (CAG)组 72例。全部病例取胃黏膜行快速尿素酶试验 (RUT)及病理特殊染色 (Gram's)检查、并行血清抗 HPIg G(EL ISA)检测 ,两项阳性者为 HP感染 ,均呈阴性者为无 HP感染 ,仅一项阳性者不计入内 ;确诊 HP感染后 ,用Western Blotting方法检测 Cag A抗体 ,根据 Cag AHP感染阳性标准分为 Cag A+ HP组及 Cag A- HP两组 ,并分别检测两组血清IL- 8、IL- 6、TNF- α含量。结果 :两组间 HP感染率差异无统计学意义 (P >0 .0 5 ) ,而 Cag A+ HP感染率及 Cag A+ HP/ HP比值 ,CAG组明显高于 AC组 (P<0 .0 1 ) ,Cag A+ HP组 IL- 8、TNF- α含量明显高于 AC组 ,而 IL- 6含量两组间差异无统计学意义 (P>0 .0 5 )。结论 :Cag A+ HP、IL - 8、TNF-α在引起胃炎进一步炎症反应及病理损害过程中可能起重要的作用 ;IL -

Objective:To study the relation between helicobacter pylori CagA on gastritis and IL 8,IL 6,TNF α in serum.Methods:120 cases were divided into two groups:control group(AC group,48 cases)and chronic gastritis group(CAG group,72 cases).All samples underwent rapid urease tests and tests of anti HPIgG in serum with ELISA and had pathology special dye.If there are two postive indexex,the corresponding cases were infected with HP.Otherwise,negative.If only one index is postive,the corresponding cases were not included in the study.After HP infection was confirmed,anti CagA's concentration was tested with Western Blotting.We divided these cases into two groups: CagA +HP and CagA -HP,and examined the concentration of IL 8,IL 6 and TNF α in serum separately. Results:The HP infection rate is insignificant between the two groups. But the infection rate of CagA +HP and rate of CagA +HP/HP,CAG group is higher than AC group significantly. The concentration of IL 8 and TNF α in CagA +HP group is higher than AC group,but the concentration of IL 6 is insignificant between the two groups.Conclusion:CagA +HP, IL 8 and TNF α play an important roles on causing advanced inflammation reaction and pathologic damage in the process of gastritis.Perhaps IL 6 does not take part in developing process of gastritis .
