依那普利对糖尿病肾病肾小球滤过率和血β_2微球蛋白的影响

The Effect of Emlapri on GFR and β_2MG in Patients with Diabetic Nephropathy

目的观察依那普利对早期糖尿病肾病的治疗效果。方法选择不同时期的糖尿病肾病病人62例,随机分为对照组和治疗组,两组同时给予常规综合治疗。对照组只给予常规治疗,治疗组在对照组的用药基础上加用依那普利常规剂量治疗,4周后测定两组病人血糖、血压、肾小球滤过率(GFR)与血β_2微球蛋白(β_2MG)的变化。结果正常白蛋白尿期中,治疗组肾小球过滤率降低非常显著,平均动脉压和血β_2微球蛋白降低显著,对照组各指标则无显著性差异;微白蛋白尿期中,空腹血糖、平均动脉压和血β_2微球蛋白降低显著,对照组各指标无差异;在临床微白蛋白期中,除治疗组血压外,两组各指标无显著性差异。结论依那普利可以有效地改善早期糖尿病肾病的肾功能,但对于晚期的肾功能未显示出明显疗效。

Objective To observe the changes of GFR and serum β2MG in patients with diabetic nephropathy (DN) treated by enalapri. Methods 62 patients with DN at three stages were divided into treament and control groups at random.Two groups were treated routinely with general treatment on DM and the treatment group was added routine dose of enalapri. After 4 weeks, FBG, Blood pressure, GFR and serum β2MG were determined. Results In normal albuminuria period, average BP and serum β2MG in treatment group were lower markedly, and GFR lower most markedly while all index in control group were not different markedly.In micro - albuminuria period, FBG, average BP, and serum β2MG in treatment group were lower markedly while that of control group were not different markedly. In clinical albuminuria period, all index except average BP in treatment group were not different markedly between both groups. Conclusion Enalapri can improve effectively the renal function of DN in earlier period, but has no special effect on renal function in late period.
