上海市某社区糖尿病患者眼部并发症患病率及相关危险因素分析

The Prevalence and Analysis of Associated Risk Factors of Ocular Complications in Diabetic Patients in A Community of Shanghai

目的了解上海市徐家汇街道社区徐汇新村站点内糖尿病患者眼部并发症的发生情况及其与血清学指标的关联程度。方法对社区内所管理的糖尿病患者开展流行病学调查,眼底筛查和血清学检查,分析眼部并发症的发生情况及其与血清学指标的关联程度。结果此次筛查无异常眼比例为65.3%,眼底表现异常者以白内障和视网膜动脉硬化最为常见(6.97%,7.77%),而明确诊断的糖尿病视网膜病变远低于国内报道(2.19%)。多因素Logistic回归分析发现年龄和糖化血红蛋白水平是糖尿病病人发生眼部并发症的独立危险因素。结论社区医生应该重视糖尿病患者眼部并发症的预防,尤其是应当严格控制血糖水平减少其波动,同时组织社区患者定期行眼部并发症筛查,建立规范的眼部并发症管理档案以期早发现,早治疗。

Objective To study the ocular complications in diabetic patients in the Xuhuinewtown,Xujiahui street at Shanghai and correlation with serological index.Methods The epidemiological investigation,eye examination and serum tests were all carried out in the diabetic patients in the community to analyze the prevalence of ocular complications and the correlation.Results Eyes without abnormalities account for 65.3%,while the most common complications are cataract and retinal artery sclerosis,which account for 6.97%and 7.77%respectively.However,the proportion of definitely diagnosed diabetic retinopathy is far from less than previously reported(2.19%).Multifactorial Logistic regression analysis found age and glycosylated hemoglobin(HbA1 c)are two independent risk factors of ocular complications with diabetes.Conclusion Prophylaxis of ocular complications should be attached importance to in diabetic patients in the community,especially giving a strict control of serum glucose levels to minimize the fluctuations,organizing the patients for a periodical eye screening,as well as establishing the specific files for documenting and managing the ocular complications,for the sake of early detecting and treating.
