经桡动脉行冠状动脉介入治疗术后桡动脉气囊压迫器不同压迫时间止血效果比较 被引量:10

摘要 目的探究经桡动脉行冠状动脉介入治疗(TRI)术后桡动脉气囊压迫器不同压迫时间的止血效果。方法选择2018年9月—2019年1月于玉环市人民医院行TRI治疗的102例患者为研究对象,并将其随机分为A组和B组,各51例。两组均在TRI术后使用桡动脉气囊压迫器对桡动脉穿刺部位压迫止血,A组术后2 h开始减压放气,每隔1 h减压1次,至8 h时完全解除压迫;B组术后4 h开始减压放气,每隔2 h减压1次,至12 h时完全解除压迫。比较两组止血效果、术后术侧肢体血氧饱和度(Sp O2)、穿刺处疼痛不适情况、术侧手掌肿胀程度及术后并发症发生情况。结果 A组患者均成功止血,B组有4例患者未能一次成功止血,A组止血成功率(100.00%)显著高于B组(92.16%),差异有统计学意义(P<0.05)。术后4 h,B组术侧肢体SpO2显著低于A组,A组舒适度显著高于B组,差异有统计学意义(P<0.05)。A组术侧手掌肿胀程度显著低于B组,术后并发症发生率显著低于B组,差异有统计学意义(P<0.05)。结论 TRI术后桡动脉气囊压迫器第一次减压时间提前至术后2 h,压迫总时间8 h,可有效止血,改善患者术侧肢体指端末梢供血,降低手掌肿胀程度和术后并发症发生率,明显增加患者舒适度。 Objective To explore the hemostatic effect of different compression time of radial artery balloon compressor after transradial intervention(TRI).Methods A total of 240 patients who underwent TRI in Yuhuan People’s Hospital from September 2018 to January 2019 were selected and randomly divided into group A and group B,with 51 cases in each group.All patients were given radial artery balloon compressor to stop bleeding at the puncture site of radial artery after TRI.Then group A began decompression and deflation at 2 h after operation,and given once decompression for every 1 h until complete compression relief at 8 h.And group B began decompression and deflation at 4 h after operation,and given once decompression for every 2 h until complete compression relief at 8 h.The hemostatic effects,oxygen saturation(SpO2)of limb at operative side,the pain and discomfort at puncture site after operation,swelling degree of palm at operative side and occurrence of postoperative complications were compared between the two groups.Results All patients in group A successfully stopped bleeding,and 4 patients in group B failed to stop bleeding successfully in one time,and the success rate of hemostasis in group A(100%)was higher than that in group B(92.16%),and the difference was statistically significant(P<0.05).At 4 hours after operation,the SpO2 of limb at operative side in group B was significantly lower than that in group A,and the comfort in group A was significantly higher than that in group B(P<0.05).The swelling degree of palm at operative side in group A was significantly lower than that in group B,and the incidence rate of postoperative complications was significantly lower than that in group B(P<0.05).Conclusion The first decompression time at 2 h after operation and 8 h total compression time of radial artery balloon compressor after TRI can effectively stop bleeding,improve the blood supply to the distal extremity of limbs,and reduce the swelling degree of palm and the incidence rate of postoperative complications,and can significantly increase patient comfort.
