绑定式医护同组管理在过敏性紫癜患儿出院后的应用价值

Application Value of Binding Medical and Nursing Group Management in Children with Allergic Purpura after Discharge

目的探讨过敏性紫癜患儿出院后使用绑定式医护同组管理应用价值。方法回顾性分析2017年1月—2018年1月出院后继续门诊随访的过敏性紫癜110例患儿临床资料,依据管理方式不同,随机分为绑定式医护同组(称观察组,60例)和常规院外延续性管理组(称对照组,50例),比较两组患儿管理前后舒适度、家长负性情绪变化、患儿遵医行为、家长配合度及管理后疾病复发率。结果患儿关节痛、恶心呕吐、皮肤瘙痒等不适感观察组明显少于对照组,比较差异有统计学意义(P<0.05);两组患儿家长经过管理后HAMD以及HAMA评分均明显下降(P<0.05),观察组患儿家长上述负性情绪评分下降水平显著高于对照组(P<0.05);管理后两组患儿遵医行为和家长配合度均明显改善,管理后观察组患儿饮食、服药依从性和家长配合度显著高于对照组(P<0.05);观察组患儿管理后疾病复发率显著低于对照组(5%vs 24.00%,χ2=8.36,P<0.05)。结论出院后过敏性紫癜患儿接受绑定式医护同组管理可显著改善患儿遵医行为,缓解家长消极情绪,并减少患儿不适感和疾病复发率,临床应用价值高。

Objective To investigate the application value of bonding management of doctor-nurse in the same group in children with allergic purpura after discharge.Methods A retrospective analysis was performed on the data of 110 allergic purpurapatients undergoing treatment who still received related continuous management after discharge during the period from January 2017 to January 2018.According to different management methods,they were divided into control group(given continuous management after discharge,n=50)and observation group(given bonding management of doctor-nurse in the same group,n=60).The comfort degrees,changes of patriarchs’negative emotions,the treatment compliance of children,the cooperationdegree of patriarchs and the recurrence rate of disease before and after management were compared between the two groups.Results The number of children with discomfort such as pain,nausea,vomiting and itchy skin was significantly reduced in the two groups after management.The number of children with discomfort in the observation group was lower than that in the control group(P<0.05).After management,the scores of HAMD and HAMA were significantly decreased in the two groups(P<0.05).The decreased level of the above negative emotion scores of the patriarchs in the observation group was significantly higher than that in the control group(P<0.05).After management,the treatment compliance of children and the cooperation degree of patriarchs in the two groups were significantly improved.After management,the children’s diet,medication compliance and cooperation degree of patriarchs in the observation groupwere significantly higher than those in the control group(P<0.05).The recurrence rate of disease in the observation group was significantly lower than that in the control group after management(5.00%vs 24.00%)(χ~2=8.36,P<0.05).Conclusion The bonding management of doctor-nurse in the same group can significantly improve the treatment compliance of children for children with allergic purpura after discharge,relieve patriarchs’negative emotions,reduce the discomfort and children’recurrence rate of disease.
