原发性肝癌介入治疗中经导管肝动脉栓塞与肝动脉灌注化疗栓塞的远期疗效比较

Long term efficacy of transcatheter arterial chemoembolization combined with transcatheter arterial chemoembolization for primary liver cancer

目的讨论原发性肝癌患者治疗中用经导管肝动脉栓塞术与肝动脉灌注化疗栓塞术的远期效果比较。方法选取2012-01-2013-06期间,笔者医院进行原发性肝癌治疗的30例患者,进行经导管肝动脉栓塞术治疗,为实验组,同一时间段在上海选取能够进行肝动脉灌注化疗栓塞术的医院30例患者,进行对比实验,比较患者治疗开始至治疗后随访结束期间,患者的生存率情况。结果根据Child-Pugh对肝硬化患者的肝脏储备功能进行量化分级(A、B、C级),当患者肝脏储备功能为C级时,实验组在治疗后随访一年、两年以及三年的生存率分为78.6%、42.8%、14.3%;对照组分为90.9%、63.6%、45.5%,两组差异具有统计学意义,P<0.05。当患者肝脏储备功能为A级或B级时,一年、两年以及三年的平均生存率分为87.5%、56.25%、37.5%;对照组分为83.95%、63.75%、35.85%,两组差异不具有统计学意义,P>0.05。结论在治疗原发性肝癌远期疗效的比较中,经导管肝动脉栓塞术与肝动脉灌注化疗栓塞术治疗生存率相似,但在肝脏储备功能严重低下时的远期治疗生存率中,肝动脉灌注化疗栓塞的治疗效果要明显好于经导管肝动脉栓塞术。

OBJECTIVE To compare the long-term results of transcatheter arterial chemoembolization with transcatheter arterial chemoembolization in the treatment of primary liver cancer.METHODS From January 2012 to June2013,30 patients with primary liver cancer underwent transcatheter arterial embolization(TACE).The experimental group was treated with transcatheter arterial chemoembolization(TACE)in the same time period in Shanghai.Perfusion chemotherapy embolization in 30 patients hospital,were compared to compare the beginning of treatment to follow-up treatment after treatment,the survival rate of patients.RESULTS According to Child-Pugh,the liver reserve function of patients with liver cirrhosis was quantitatively graded(A,B,C).When the liver function was C,the experimental group was followed up for one year,two years and three years The survival rates of the two groups were 78.6%,42.8% and14.3% respectively.The control group was 90.9%,63.6%and 45.5%.The difference was statistically significant(P<0.05).The average survival rates of one year,two years and three years were 87.5%,56.25%and 37.5%respectively.The control group had 83.95%,63.75% and 35.85% Group differences were not statistically significant,P>0.05.CONCLUSIONS In the comparison of the long-term efficacy of primary hepatic carcinoma,the survival rate of transcatheter hepatic arterial embolization and hepatic arterial chemoembolization is similar,but in the long-term survival rate of patients with severe hepatic reserve dysfunction Arterial infusion chemotherapy embolization to be significantly better than transcatheter hepatic artery embolization.
