多次输血的恶性肿瘤患者不同配型方法输注血小板时的有效率比较分析

Comparative analysis of the efficiency of transfusion of platelets with different matching methods in patients with multiple transfusion

目的综合分析多次输血的恶性肿瘤患者实施不同配型方式的血小板输注的应用效果。方法选取2017-1-2017-12需要进行血小板输注的恶性肿瘤患者173例作为研究对象,将其随机分成对照组和观察组,其中对照组中有患者86例,观察组87例;运用固相凝集的方法对两组进行血小板抗体检测,并统计好两组患者血小板抗体的阳性比率。随机抽选同型ABO血型的血小板供应者对对照组中的患者进行血小板输注;而对观察组中患者血小板的输注则采用交叉配型的方式,结合患者自身情况以及依据实验结果选择相适应的血小板进行输注。分别在输注前以及输注24h之后对患者的前臂静脉血小板检测进行计数,并且采用相应的计数软件对患者血小板计数进行增高指数计算(CCI),最后依照具体数据对血小板输注效果进行评判。结果 173例患者中有68例血小板抗体阳性,其中患者包括36例观察组,32例对照组。观察组患者CCI值明显高于对照组,差异有统计学意义(P<0.05);观察组患者血小板输注效率高于对照组,差异有统计学意义(P<0.05)。结论对多次输血的恶性肿瘤患者实施不同配型方式的血小板输注的应用效果明显。

OBJECTIVE To analyze the application effect of platelet transfusion with different matching modes in malignant tumor patients with multiple transfusion.METHODS From January 2017 to December 2017,173 patients with malignant tumors needed to be transfused in our hospital were divided into control group and observation group,including86 cases in the control group and 87 cases in the observation group.The two groups of platelet antibodies were detected by solid phase agglutination,and the positive rate of platelet antibody in two groups was statistically good.Platelet providers with random selection of the same type of ABO blood group were transfused to the patients in the control group,while the platelet transfusion in the observation group was cross matching,combined with the patient’s own situation and the selection of the platelets adapted to the experimental results.The patient’s forearm venous platelets were counted before infusion and after infusion of 24 h,and the corresponding counting software was used to calculate the platelet count(CCI).Finally,the platelet transfusion effect was evaluated according to the specific data.RESULTS Among the 173 patients,68cases were positive for platelet antibodies,including 36 cases in the observation group and 32 cases in the control group.The CCI value of the patients in the observation group was significantly higher than that of the control group.The difference between the two groups was statistically significant(P<0.05),and the platelet transfusion efficiency in the observation group was significantly higher than that in the control group.The difference was significant(P<0.05).CONCLUSIONS The application of platelet transfusion with different matching modes is obvious in patients with multiple transfusion.
