The role of MRI in predicting the texture of pituitary adenomas: a comparative study on pathological analysis

摘要 目的通过分析与垂体腺瘤质地相关的因素,探讨MRI在预判垂体腺瘤质地中的作用,为术前诊断及手术方式的选择提供参考。方法回顾性分析本院2016年8月—2018年3月收治的垂体腺瘤患者临床资料,将术前MRI影像学表现、术中所见瘤体质地及术后病理学方法测定的肿瘤胶原纤维含量这三个因素的相关性进行分析。结果根据术中所见,MRI中T1WI肿瘤信号与垂体腺瘤质地无明显相关性(P>0.05); MRI中T2WI肿瘤与白质信号比值及与脑桥信号比值和肿瘤质地的相关性明显(P<0.05),差异有统计学意义。质地软的肿瘤组织的胶原含量明显低于质地硬的肿瘤组织的胶原含量(P<0.05),质地不同的肿瘤所含胶原含量有显著的统计学差异(P<0.05)。此外,MRI中T2WI肿瘤信号与白质信号比值与不同质地的胶原含量呈显著负相关(P<0.01)。结论术前垂体腺瘤MRI的T2WI信号强度对于手术方式的选择具有重要的预测价值。 Objective To analyze the factors related to the texture of pituitary adenoma and to explore the role of MRI in predicting the texture of pituitary adenoma,and to provide reference for preoperative diagnosis and choice of operation mode.Methods The clinical data of 50 patients with pituitary adenoma treated in the first affiliated hospital of Bengbu Medical College were analyzed retrospectively.The MRI scans,tumor texture types and the content of tumor collagen fibers were analyzed. Results There was no significant correlation between T1 WI tumor signal and pituitary adenoma texture( P> 0.05). The ratio of T2 WI tumor to white matter signal and pons signal and tumor texture was significantly correlated( P < 0. 05). Collagen content in soft tumor tissue was significantly lower than that in hard tumor tissue( P < 0. 05),and there was significant difference in collagen content in different texture( P<0.05).In addition,the ratio of T2 WI signal to white matter signal was negatively correlated with collagen content of different textures on MRI scans( P<0.01). Conclusions Preoperative T2 WI signal intensity of pituitary adenoma has an important predictive value for the choice of surgical modes.
