摘要
目的通过比较原发性高血压患者自动测量血压与动态血压值,探讨自动测量血压(ABP)在高血压管理中的应用价值。方法选取2015年8月—2017年2月在本院拟行动态血压监测(ABPM)的高血压患者67例,在患者行ABPM之前利用水银血压计测得传统诊室血压(OBP),在行ABPM之后嘱受试者自行前往住所附近配有自动血压测量仪的医疗机构或药店测得ABP。对测得血压值进行配对t检验和Pearson相关分析。结果通过比较58例高血压患者的OBP、ABP以及ABPM发现ABP与日间ABPM的差值均值均小于3/2.5 mm Hg,该差异明显小于OBP与日间ABPM的6.5/4.2 mm Hg;另外,随着对自动血压测量设备和周围环境的熟悉,ABP与ABPM daytime差值也呈减少趋势。进一步的相关性分析显示ABP与日间ABPM显著相关(不同三天测得ABP值与日间ABPM相关系数分别是0.349/0.571、0.491/0.414、0.511/0.330)。结论 ABP与ABPM有较好的相关性,可以作为ABPM和家庭自测血压有效补充,在高血压管理中发挥作用。
Objective To explore the value of ABP in hypertension management by comparing automated blood pressure(ABP)and ambulatory blood pressure(ABPM)values in patients with hypertension.Methods Sixty seven hypertensive patients who scheduled for ABPM in our hospital during August and February 2017 were selected.Traditional office blood pressure(OBP)was measured using a mercury sphygmomanometer before the patient had ABPM.After completing ABPM,we suggest the patients to measure ABP themselves in medical institution or pharmacy nearby home where own automatic blood pressure device.The blood pressure values were analyzed using paired t-test and the Pearson correlation analysis.Results By comparing the OBP,ABP,and ABPM in 58 hypertensive patients,the difference between ABP and ABPM in daytime was less than 3/2.5 mm Hg,which were significantly less than 6.5/4.2 mm Hg between OP and ABPM in daytime.In addition,with the patients familiar with the ABP equipment and the surrounding environment,the difference between ABP and ABPM in daytime also showed a decreasing trend.Further correlation analysis showed that ABP was significantly associated with daytime ABPM(ABP values measured in different days and day ABPM correlation coefficient were 0.349/0.571,0.491/0.414,0.511/0.330).Conclusions ABP could be used as an effective supplement for ABPM and self measured blood pressure,and plays a role in hypertension management.
