古琴音乐疗法对轻中度阿尔茨海默病患者睡眠影响的研究

Study on the influence of Guqin Music Therapy on sleep in patients with mild to moderate Alzheimer disease

目的初步探索古琴音乐疗法对轻中度阿尔茨海默病(AD)患者睡眠情况的影响。方法选择2016年9月—2018年9月本院收治的50例睡眠障碍的轻中度AD患者作为研究对象,在进行常规治疗的基础上给予古琴音乐疗法,每次60分钟,每周5次,共12周。分别在基线、治疗第2、4、9、11周末进行睡眠障碍评定量表(SDRS)、汉密尔顿焦虑量表(HAMA)评估,比较不同时间点患者的睡眠质量差异。结果相比基线时的评分,治疗第2、4、9、11周末SDRS及HAMA的评分均出现明显下降,差异有统计学意义(P<0.05)。在治疗第4周末效果较明显,之后睡眠障碍及焦虑评分逐渐趋于稳定。另外,本研究中有2例患者的抗精神病药物及3例患者的苯二氮艹卓类药物较基线有减量。结论古琴音乐疗法所拥有的独特音韵,可能是改善轻中度AD患者睡眠障碍的有效辅助治疗方法。

Objective To explore the influence of Guqin music therapy on sleep disorder in patients with mild to moderate Alzheimer disease(AD).Methods A total of 50 patients with mild to moderate AD,suffered from sleep disorders,were collected.They were given Guqin music therapy on the basis of routine treatment,60 minutes each time,5 times a week for 12 weeks.Sleep Disorder Rating Scale(SDRS)and Hamilton Anxiety Scale(HAMA)were used to assess the sleep quality before treatment,2 nd,4 th,9 th and 11 th week of treatment respectively,and the differences of sleep quality at different time points were compared.Results The scores of SDRS and HAMA at the 2 nd,4 th,9 th and 11 th weekends were significantly decreased compared those before treatment(P<0.05).The therapeutic effect was relatively obvious at the 4 th weekend of treatment,and then the scores of sleep and emotional symptoms gradually stabilized.Among them,two patients received antipsychotic drugs and 3 patients use benzodiazepines achieved dosage reduction.Conclusions The unique rhyme of Guqin music therapy might be an effective adjuvant therapy to improve sleep disturbance in patients with mild to moderate AD.
