锁定加压钢板联合皮质外骨桥技术治疗肱骨干骨折术后萎缩性骨不连的疗效探讨 被引量:2

摘要 目的 :探讨用锁定加压钢板联合皮质外骨桥技术治疗肱骨干骨折术后萎缩性骨不连的临床疗效。方法 :将2014年1月至2018年12月雅安职业技术学院附属医院收治的40例肱骨干骨折术后萎缩性骨不连患者作为研究对象。将其随机分为研究组和对照组。为对照组患者应用锁定加压钢板进行治疗,为研究组患者用锁定加压钢板联合皮质外骨桥技术进行治疗。然后对比两组患者的临床疗效。结果 :治疗后,两组患者骨折愈合的时间、并发症的发生率相比,P>0.05;研究组患者的Neer肩关节功能评分、Mayo肘关节功能评分、Neer肩关节功能评分的优良率、Mayo肘关节功能评分的优良率均高于对照组患者,P <0.05;研究组患者中再次发生骨不连患者的占比低于对照组患者,P <0.05。结论 :用锁定加压钢板联合皮质外骨桥技术治疗肱骨干骨折术后萎缩性骨不连可有效地改善患者肘关节及肩关节的功能,降低其再次发生骨不连的风险。 objective:to investigate the clinical effect of locking compression plate combined with cortical external bone bridge in the treatment of atrophic nonunion after humeral shaft fracture.Methods:40 cases of postoperative atrophic nonunion of humerus shaft fractures admitted to the affiliated hospital of ya‘an vocational and technical college from January 2014 to December 2018 were studied.They were randomly divided into study group and control group.Patients in the control group were treated with locking compression plate,and patients in the study group were treated with locking compression plate combined with extracortical bony bridge technology.Then the clinical efficacy of the two groups was compared.Results:after treatment,the time of fracture healing and the incidence of complications in the two groups were compared,P>0.05.The excellent and good rates of Neer shoulder joint function score,Mayo elbow joint function score,Neer shoulder joint function score and Mayo elbow joint function score in the study group were all higher than those in the control group(P<0.05).The proportion of patients with recurrent nonunion in the study group was lower than that in the control group(P<0.05).Conclusion:the treatment of atrophic nonunion after humeral shaft fracture with locking compression plate combined with cortical bone bridge can effectively improve the function of elbow and shoulder joints and reduce the risk of recurrence of nonunion.
