高危型HPV感染与宫颈癌前病变及宫颈癌的相关性 被引量:4

摘要 目的 :探讨高危型HPV(人乳头瘤病毒)感染与宫颈癌前病变及宫颈癌的相关性。方法 :将2015年1月至2017年12月期间在雅安市妇幼保健计划生育服务中心就诊的134例宫颈癌前病变患者、135例宫颈癌患者和同期在该中心进行健康体检的134人作为研究对象。将其中的134例体检者作为对照组,将其中的134例宫颈癌前病变患者作为CINⅠ~Ⅲ级组,将其中的135例宫颈癌患者作为宫颈癌组。对三组研究对象均进行宫颈液基薄层细胞学检测和HPV-DNA检测。然后,比较三组研究对象高危型HPV感染的阳性检出率,并比较三组高危型HPV感染者中各型HPV感染者的占比。结果 :宫颈癌组患者和CINⅠ~Ⅲ级组患者高危型HPV感染的阳性检出率均高于对照组体检者高危型HPV感染的阳性检出率,宫颈癌组患者高危型HPV感染的阳性检出率高于CINⅠ~Ⅲ级组患者高危型HPV感染的阳性检出率,P <0.05。在三组高危型HPV感染者中HPV16感染者的占比均高于HPV33感染者、HPV18感染者、HPV52感染者和其他高危型HPV感染者的占比,P <0.05。结论 :三组研究对象高危型HPV的阳性检出率由高至低依次为宫颈癌患者、宫颈癌前病变患者和体检者,说明高危型HPV感染可导致宫颈病变。高危型HPV感染与宫颈癌前病变及宫颈癌密切相关。 objective:to investigate the correlation between high-risk HPV(human papillomavirus)infection and cervical cancer prelesions and cervical cancer.Methods:134 cases of pre-cervical cancer patients,135 cases of cervical cancer patients and 134 people who underwent physical examination in the center during the period from January 2015 to December 2017 were studied.Will be one of the 134 cases of medical examination as a control,will be one of the 134 patients with cervical cancer lesion before as CINⅠ~Ⅲlevel group,will be as cervical cancer group with 135 cases of cervical cancer patients.Cervical fluid-based thin layer cytology and HPV-DNA detection were performed on all three groups.Then,the positive detection rates of high-risk HPV infection in the three groups of study subjects were compared,and the proportion of each type of HPV infection in the three groups of high-risk HPV infection was compared.Results:the patients with cervical cancer group and CINⅠ~Ⅲlevel set of high-risk type HPV infection in patients with positive detection rate were higher than control group in physical examination is a high-risk type HPV infection positive detection rate,positive rate of high-risk type HPV infection in patients with cervical cancer group was higher than that of CINⅠ~Ⅲlevel group of patients with high-risk type HPV infection positive detection rate,P<0.05).In the three groups of high-risk HPV infection,the proportion of HPV16 infection was higher than that of HPV33 infection,HPV18 infection,HPV52 infection and other high-risk HPV infection,P<0.05.Conclusion:the positive detection rate of high-risk HPV in the three groups of study objects was successively from high to low for cervical cancer patients,cervical precancerous lesions patients and physical examination patients,indicating that high-risk HPV infection can cause cervical lesions.High risk HPV infection is closely related to the precancerous lesions and cervical cancer.
