用奥曲肽和普萘洛尔治疗肝硬化食管胃底静脉曲张破裂出血的效果探讨

The effect of octreotide and propranolol in the treatment of esophageal and gastric varicose hemorrhage with cirrhosis

目的:研究用奥曲肽和普萘洛尔治疗肝硬化食管胃底静脉曲张破裂出血的临床效果。方法:将2017年2月至2018年12月期间河池市人民医院收治的52例肝硬化食管胃底静脉曲张破裂出血患者分成研究组和对照组,每组各有26例患者。对两组患者的病情均进行胃镜检查。在此基础上,为研究组患者使用奥曲肽和普萘洛尔进行治疗,为对照组患者使用普萘洛尔进行治疗。然后,比较两组患者止血的时间、输血量及血清丙氨酸氨基转移酶的水平和血清天门冬氨酸氨基转移酶的水平。结果:治疗后,研究组患者止血的时间短于对照组患者,其输血量少于对照组患者,其血清丙氨酸氨基转移酶的水平与血清天门冬氨酸氨基转移酶的水平和不良反应的发生率均低于对照组患者,P <0.05。结论:用奥曲肽和普萘洛尔治疗肝硬化食管胃底静脉曲张破裂出血的效果显著,可促进患者肝功能的恢复,缩短其止血的时间,减少其输血量,降低其不良反应的发生率。

objective:to study the clinical effect of octreotide and propranolol in the treatment of esophageal and gastric varicose hemorrhage with cirrhosis.Methods:fifty-two patients with esophageal and gastric varicose hemorrhage with cirrhosis admitted to hechi people’s hospital from February 2017 to December 2018 were divided into study group and control group,each group had 26 patients.Gastroscopy was performed on the patients in both groups.On this basis,patients in the study group were treated with octreotide and propranolol,and patients in the control group were treated with propranolol.Then,the hemostasis time,blood transfusion volume and serum alanine aminotransferase level and serum aspartate aminotransferase level were compared between the two groups.Results:after treatment,the hemostasis time of the study group was shorter than that of the control group,the blood transfusion quantity was less than that of the control group,the serum alanine aminotransferase level,the serum aspartate aminotransferase level and the incidence of adverse reactions were lower than that of the control group,P<0.05.Conclusion:octreotide and propranolol are effective in the treatment of esophageal and gastric varicose bleeding in patients with cirrhosis,which can promote the recovery of liver function,shorten the time of hemostasis,reduce the amount of blood transfusion and reduce the incidence of adverse reactions.
