多层螺旋CT在脊柱损伤诊断中的应用 被引量:9

摘要 目的 :评价多层螺旋CT(MSCT)在脊柱损伤诊断中的应用价值。方法 :对 36例脊柱损伤患者行多层螺旋CT扫描 ,利用Syngo 3D软件进行多平面重建 (MPR)、表面遮盖显示 (SSD)、最大密度投影 (MIP)以及用Fly -Through作仿真内窥镜观察 ,全方位对病变部位、范围、解剖结构及邻近组织关系进行比较分析。结果 :全组病例的薄层重建及三维重建显示椎体骨折 36例 ,椎体骨折伴椎管狭窄 13例 ,小关节脱位 12例。多层螺旋CT能很好地显示相应的骨折、脱位及椎管情况 ,尤其是MPR及MIP对椎体骨折显示率为 10 0 % ;仿真内窥镜能多角度观察病变椎管内形态及狭窄状况。结论 :多层螺旋CT尤其是MPR、MIP及仿真内窥镜的应用能直观、立体、清晰、多角度地显示脊柱损伤 ,具有较高的临床应用价值。 Aim: To evaluate the clinical application of multi-slice helical CT(MSCT) to diagnosing of spinal column trauma. Methods: MSCT was performed in 36 cases of spinal column trauma, we used the Syngo 3D softwares to perform multiplanar reconstruction(MPR),shaded surface display(SSD),maximum intensity projection(MIP),Fly-through software for virtul endoscopy observation,all directions to compare and analyse the injured dimensions, anatomical structure and near tissue. Results: In all cases, the excellent thin layer reconstructed combined with multiplanar reconstructed and 3D reconstructed images,MSCT were capable of displaying well the whole picture of spinal fracture, dislocation and spinal canal. Rate of displaying spine fractures with MPR and MIP is 100%. Virtul endoscopy was capable of displaying multiangularly spinal canal injury and structure. Conclusion: Multi-slice helical CT images, especially MPR,MIP and VE imsges can directly, stereoscopically and multi-angularly display trauma of spinal column. It is of high value in clinical practice.
