卵巢恶性肿瘤保留生育功能手术后化疗对卵巢功能的影响 被引量:27

摘要 目的探讨卵巢恶性肿瘤患者接受保留生育功能的手术后化疗对卵巢内分泌功能的影响。方法卵巢恶性肿瘤患者,观察保留生育功能手术后化疗对其月经情况及血清性激素水平变化的影响。结果15例保留生育功能的手术后化疗患者中13例出现月经异常(86.7%),其中12例发生停经(80%),1例为月经减少(6.7%)。发生停经的患者,化疗结束后月经恢复;月经恢复时间在化疗结束后2个月左右。发生停经的患者在停经期间血清黄体生成素(LH)和促卵泡激素(FSH)明显升高,雌二醇(E2)明显降低;雄、孕激素无明显变化;结论年轻妇女卵巢恶性肿瘤,行保留生育功能的手术后化疗,可发生卵巢功能损害,发生率超过80%;卵巢功能损害表现为血清促性腺激素浓度升高,雌激素浓度降低,停经或月经异常;化疗导致的停经是可逆的。 To determine the effect of chemotherapy on ovarian endocrine function and menstruation.Methods Menstruation and serum luteinizing hormone(LH),follicle-stimulating hormone(FSH),and estradiol(E 2 )levels were observed when the patients with ovarian cancer undergoing one side ovarecto-my were followed by chemotherapy.Results Abnormal menstruation occurred in86.7%cases,including80%amenorrhea and6.7%oligomenstruate.Menstruation resumed around2months after chemotherapy.Serum LH,FSH levels rose and E 2 level declined during amenorrhea.No significant change was observed in progesterone or testosterone levels.Conclusions Ovary function impairment may occur in ovarian cancer patient treated by one side ovarectomy followed by chemotherapy.Serum LH,FSH,and E 2 levels change and abnormal menstruation are two common manife stations.However,chemotherapy-related amenorrhea is reversible.
