摘要
目的 增殖细胞核抗原 (PCNA)与抑癌蛋白 (P53蛋白 )在先天性胆管扩张症 (CBD)胆管粘膜上皮细胞中的表达 ,了解上皮细胞的增殖分化情况 ,探讨可能的胆管癌变机制。方法 应用免疫组化方法检测胆管上皮中 PCNA与 P53蛋白表达 ,以胆管癌和正常胆总管组织作为对照 ,进行相关关系分析。结果 CBD的 PCNA与P53蛋白表达阳性率为 (4 3.5± 2 5 .5 ) %和 (2 8.5± 2 0 .0 ) % ,正常胆管表达阳性率为 (7.4± 5 .0 ) %和 (1.0± 2 .5 ) % ,胆管癌为 (74 .9± 18.9) %和 (6 4 .5± 16 .8) %。三者之间差别有极显著差异。 PCNA在化生部位表达率增高。结论 CBD胆管上皮细胞的增殖分化处于高增殖状态 。
Objective To study the cell proliferation and transformation and discuss the effects of carcinogenic factors in congenital biliary dilatation(CBD) Methods Using 12 cases of biliary cancer and 11 cases of normal bile duct tissue as control,the PCNA and P 53 protein were examined in formalin fixed,paraffin embeded material of epithelium of 56 cases of CBD by immunohistochemical method Results PCNA and P 53 protein labeling index was(43 5±25 5)% and(28 5±20 0)% in 56 cases of biliary dilatation,(7 4±5 0)% and (1 0±2 5)% in 11 cases of normal bile duct and(74 9±18 9)% and(64 5±16 8)% in 12 cases of biliary cancer respectively and there were significant difference among the three groups(P<0 01) The expression of PCNA and P 53 protein showed a highly correlation with each other PCNA expression was much higher in the position of metaplasia Conclusion The higher proliferation of dilated bile duct epithelium in CBD maybe a cause lead to carcinogenisis
