小肝癌的Gd-DTPA动态增强MRI诊断

Gd-DTPA Dynamic Enhanced MRI Diagnosis on Small Hepotacellar Carcinomas

目的 :探讨 Gd- DTPA动态增强 MRI对小肝癌 (SHCC)的诊断价值。方法 :回顾性分析 2 4例经临床和病理证实的小肝癌 MR检查资料。结果 :平扫 T1 WI多呈低信号、少数等和高信号 ,T2 WI多呈高信号 ,18.5 2 %可见假包膜。Gd- DTPA动态增强多血供癌灶 19个 ,时间 -信号强度曲线表现为速升速降 ,轻度延迟强化 ;少血供癌灶 8个 ,时间 -信号强度曲线表现为缓升 ;5 2 %可见假包膜。MRI平扫与 Gd- DTPA动态增强对 SHCC诊断的准确率分别为 74 %和 92 .6% (P<0 .0 5 )。结论 :Gd- DTPA动态增强 MRI能显著提高对 SHCC诊断准确性。

Purpose To study the diagnostic value of dynamic gadolinium enhanced MR imaging in patients with small hepotacellar carcinoma (SHCC). Methods The examination data of 24 patients with SHCC proved by pathology clinic were reviewed retrospectively. Results On plain scan, most SHCC showed hypodensity on T 1WI and hyperdensity on T 2WI, a few showed isodensity or hyperdensity on T 1WI. Pseudocapsule can be detected in 18.52 % patients. On Gd DTPA dynamic enhanced MR imaging, 19 SHHCs were hypervascular, the time intensity curve presented slow fast up and down, on delay phase, showed slight delayed enhancement. 8 SHCCs were hypovascular, and the time intensity curve present slow up. Pseudocapsule can be detected in 52 % patients, The diagnostic accuracy of Gd DTPA dynamic enhanced MR imaging on SHCC was 92.6 % and the traditional MR plain scan (T 1WI and T 2WI)was 74 %(P<0.05). Conclusion Gd DTPA dynamic enhanced MR imaging can improve the diagnostic accuracy significantly on SHCC.
