CT仿真胃镜的临床应用研究

Reseach of Clinical Application of CT Virtrual Gastroscopy

目的 :研究 CT仿真胃镜 (CT virtual gastroscopy,CTVG)在胃部病变诊断中的使用价值 ,应用方法及镜下表现。方法 :使用 Hispeed FX/ i扫描机 ,扫描层厚 3.0～ 5 .0 mm,螺距 1.0～ 1.5 ,重建间隔 1.0～ 2 .0 m m,应用 Navi-gator sm ooth功能对 2 1例病人及志愿者进行了 CTVG成像。结果 :通过选择不同的观察方位 ,角度和阈值 ,CTVG能清楚显示靶器官内正常解剖结构的立体影像和病变的部位、大小、形态及其侵犯范围。 CTVG对胃部病变形态的显示与FG和 /或标本显示具有良好的对应性。CTVG显示病变的准确率达 95 .2 %。结论 :CTVG作为一种安全、可靠、无痛苦新的检查方法 ,可以作为 FG的有效补充手段 ,有较好的临床应用价值。

Objective To investigate the procedure, imaging features and clinical significance of CT virtual gastroscopy. Methods nineteen patients and two normal volunteers were detected with volum scanning by helical CT (Hispeed Fx/i, GE).The CT scanning conditions were collimating width 3~5 cm, pitch 1.0~1.5, the raw data of CT volum scan was reconstructed in overlapping rate 33 %~67 %. Volumetric data were analysed by the navigator soft ware and along the longitudinal axis of the tissue using the soft ware fly though. CTVG images were observed. The diagnosis of all cass were proved by surgery or fiberoptic gastroscopy biopsy. Results The appearances of the inner lesion and normal wall on CTVG of the gaster were smililar to those on fiberoptic gastroscopy. Additionally, the lesion distal to obstruction could also be evaluated by CTVG, But the CTVG could not evaluated mucosa lesion and make histologic diagnosis. Conclusion CTVG is a new, non invasive method for demostrating the structure of normal and abnormal gaster, and it is a good complementory method to conventional fiberoscopitic gastroscopy.
