保留二尖瓣和瓣下结构的瓣膜置换术18例

Mitral Valve replacement with preservation of subvalular apparatus in 18 patients

目的 报告保留二尖瓣和瓣下结构的瓣膜置换术的临床经验和效果。方法 1993年 1月至 2 0 0 2年 2月 ,我们共施行保留二尖瓣和瓣下结构的瓣膜置换术 18例 ,分析其术前、术后X线胸片、超声心动图资料及手术效果。结果 本组无手术死亡 ,2例出现低心输出量综合征。术后心胸比率明显缩小 (P <0 .0 1) ,左心室舒张末径显著缩小 (P <0 .0 5 ) ,平均左心室射血分数明显降低 (P <0 .0 1) ,心功能明显改善 ,手术效果良好。结论 保留二尖瓣和瓣下结构的瓣膜置换术安全、易行 ,更有利于改善左心功能 。

Objective To report the effective and clinical experience of mitral valve replacement with preservation of subvalvular apparatus.Methods To analyze the chest x-ray, cardiac ultrasound and the effective of surgery in 18 patients who were undergone MVRP from January 1993 to February 2002.Results There was no operative mortality. Low output syndrome occurred in two patients. After surgery, the cardiac function parameter (FS, SD, EF) was improved significantly. The surgery was effective. Conclusion MVRP is a feasible, effective and safe technique; it can significantly improve left cardiac function.
