青年脑卒中2359例全国性多中心发病相关因素调查(英文)

A nation-wide multicentrical related factor investigation in 2359 cases of young stroke

目的调查中国青年脑卒中发病相关因素,寻求青年脑卒中一级预防的个体化最佳效果。方法全国18个省、市、自治区36家医院收集1996-01-01/2000-12-31所有脑卒中住院患者64558例,其中18～45岁青年脑卒中患者6305例,抽取2359例统计其发病年龄、性别,是否合并有原发性高血压、心脏病、糖尿病等因素,有无吸烟、饮酒以及临床体征、影像学检查、血脂、临床诊断和转归等情况。结果反馈调查2359例中,平均年龄40岁(18～45岁),男女比例为2.63:1,男性占77.70%。40～45岁者占青年卒中总数的62.40%,伴有原发性高血压38.02%(897/2359),冠心病7.42%(175/2359),糖尿病4.58%(108/2359),高血脂4.58%(108/2359);家族史中有高血压15.81%(373/2359),有脑卒中12.04%(284/2359),有糖尿病1.74%(41/2359)。结论青年卒中在中国并不少见;男性脑卒中患者居多数;原发性高血压及家族史原发性高血压是青年卒中最显著的危险因素。

Aim To investigate the related factors to young stroke in our country and to explore the best individualized effect for primary prevention of young stroke.Methods 64 558 in patents came from 36 hospitals in 18 provinces from January 1996 to December 2000 among which were 6 305 young stroke patients between 18- 45 years old and 2 359 patients were drawn out.Age,sex,whether complicated with hypertension,heart disease,diabetes,whether smoking,drinking and clinical signs,imaging examination,blood lipid,clinical diagnosis ,prognosis and other conditions were collected.Results In 2 359 patients,the mean age was 18- 45 years and the ratio of male to female was 2.63:1 with male patients accounting for 77.70% .In populations 40- 45 years old,patients with young stroke accounted for 62.40% ,38.02% ( 897/2 359) complicated with hypertension, 7.42% ( 175/2 359) with coronary heart disease, 4.58% ( 108/2 359) with diabetes, 4.58% ( 108/2 359) with hyperlipemia;In family history,hypertension was found in 15.81% (373/2 359),stroke in 12.04% ( 284/2 359) , diabetes in 1.74% (41/2 359).Conclusion Young stroke is not rare in our country;male patients accounted for the most;hypertension and hypertension family history are the risk factors for young stroke.
