桥小脑角区肿瘤术中面神经功能监护的应用 被引量:2

摘要 目的研究桥小脑角区肿瘤术中应用面肌肌电图(EMG)监护在面神经保留中的作用。方法对32例桥小脑角区肿瘤术中采用NIM-Pulse型神经监护仪行面肌EMG记录监测,术中常用电流为0.1~0.6 mA。应用单极的刺激器进行面神经定位。结果本组肿瘤全切除29例,次全切除3例,无手术死亡。面神经全部解剖保留。刺激值为0.1~0.2 mA 24例病人,H-B评分均为Ⅰ~Ⅱ级;0.3~0.4 mA 5例中,Ⅰ~Ⅱ级3例,Ⅲ~Ⅳ级2例;0.5~0.6 mA 3例中,Ⅲ~Ⅳ级2例,Ⅴ级1例。结论桥小脑角区肿瘤术中应用面肌EMG监护,可显著提高面神经的解剖和功能保留。 Objective To study the effect of facial electromyography (EMG) on facial nerve anatomy and function preservation during the surgery for the tumors in the cerebellopontine angle.Methods Facial EMG was performed during the surgery in 32 patients CPA tumors.A quantitative analysis of values of evoked EMG parameters was performed.The stimulation intensity was between 0.1mA and 0.6mA.Results Total tumor removal was achieved in 29 cases,subtotal removal in 3 cases.No patient died of the surgery.The facial nerve was anatomically preserved in all the 32 patients.The facial nerve function was assessed by House-Brackimann (H-B) scale 1 week after the surgery.The facial nerve function was H-B grades Ⅰ~Ⅱ in 24 patients stimulated by 0.1~0.2mA;of 5 patients stimulated by 0.3~0.4mA 3 had facial nerve function of H-B grade Ⅰ~Ⅱ and 2 grade Ⅲ~Ⅳ;of 3 patients stimulated by 0.5~0.6mA, 2 grades Ⅲ~Ⅳ,1 grade Ⅴ.Conclusion The preservation of facial nerve anatomy and function can be significantly improved by facial EMG monitoring used during the surgery in the patients with CPA tumors.
