海军陆战队员高温强训和野外生存训练后血液流变学和凝血系统的变化(英文) 被引量:7

摘要 目的探讨海军陆战队员在高温强训和野外生存训练后血液流变学和凝血系统的变化,为制定科学合理的卫生保障措施提供理论依据。方法海军陆战队员151名,其中52名参加为期1周的高温强训,43名参加野外生存训练20d,56名为陆勤人员作为对照。使用凝血分析仪、血液流变仪检测高温强训前、高温强训后、野外生存训练后的血液流变学和凝血系统相关指标。结果高温强训后,血液流变学指标:全血低切黏度、全血中切黏度、全血高切黏度、红细胞压积、红细胞沉降率、全血低切还原黏度、全血中切还原黏度、全血高切还原黏度、红细胞电泳指数分别为(9.3±0.5),(5.5±0.3),(4.5±0.2)mPa·s,(48.5±3.7)%,(7.5±1.5)mm/h,(20.5±4.1),(10.2±1.7),(7.8±2.1)mPa·s,6.0±1.8,与对照组比较,差异有显著性意义(t=2.09~3.61,P<0.01)。血沉方程K值、红细胞聚集指数、红细胞刚性指数、红细胞变性指数分别为61.8±12.5,2.6±0.4,5.3±1.2,0.9±0.1均比对照组显著增高(t=2.09~2.92,P<0.05)。野外生存训练后全血低切黏度、血浆黏度分别为(8.8±0.4),(1.2±0.1)mPa·s(t=2.32,2.45,P<0.05);红细胞沉降率、红细胞电泳指数分别为11.1±1.8,6.3±2.1,此4项均较对照组有显著升高(t=3.42,3.07,P<0.01)。 AIM:To observe the alterations of hemorheology and coagulation system in members of marine corps after hyperthermia heavy training and field survival training and to provide theoretical basis for formulation of reasonable and scientific health and safeguard measurements.METHODS:One hundred and fifty one members of marine corps were observed among which 52 members participated in hyperthermia heavy training for 1 week,43 members participated in field survival training for 20 days and other 56 rear service members as control.Hemorheology and coagulation system related indexes before and after training were detected with coagulation analyzer and hemorheometer.RESULTS:Compared with control,the hemorheological indexes:whole blood low shearing viscosity,whole blood middle shearing viscosity,whole blood high shearing viscosity,packed cell volume,erythrocyte sedimentation rate,whole blood low shearing reduction viscosity,whole blood middle shearing reduction viscosity,whole blood high shearing reduction viscosity,erythrocyte electrophoresis exponent was( 9.3± 0.5) mPa· s,( 5.5± 0.3)mPa· s,(4.5± 0.2)mPa· s,( 48.5± 3.7)% ,( 7.5± 1.5)mm/h,( 20.5± 4.1) mPa· s,(10.2± 1.7) mPa· s,( 7.8± 2.1) mPa· s,6.0± 1.8,respectively (t=2.09- 3.61, P< 0.01).Sedimentation equation K value,erythrocyte aggregation exponent,erythrocyte rigidity exponent,erythrocyte degeneration exponent was 61.8± 12.5,2.6± 0.4, 5.3± 1.2,0.9± 0.1,respectively,there was a significant difference between two groups(t=2.09- 2.92, P< 0.05).All these 13 items increased significantly.After field survival training whole blood low shearing viscosity,plasma viscosity was ( 8.8± 0.4),( 1.2± 0.1) mPa· s(t=2.32,2.45, P< 0.05) .Erythrocyte sedimentation rate,erythrocyte electrophoresis exponent was ( 11.1± 1.8) mm/h, 6.3± 2.1(t=3.42,3.07, P< 0.01) ,which increased significantly.After hyperthermia heavy training, prothrombin time(PT),activated partial thromboplastin time(APTT)( 11.4± 0.2) , ( 27.7± 1.0) s (t=2.87, 3.08,P< 0.01) ,which decreased obviously.Fibrinogen(FIB),platelet(PLT) was ( 3.5± 0.3)g/L, ( 265.6± 18.6)× 109/L, t=2.15,2.58, P< 0.05) ;Fibrin degradation product(FDP),D dimer was ( 7.9± 0.4), ( 335.9± 15.4) mg/L (t=3.22,2.91, P< 0.01) ,which increased significantly compared with control group.However,after field survival training, PT( 12.4± 0.3) s(t=2.11, P< 0.05) ,APTT( 34.1± 4.6) s(t=2.97, P< 0.01) increased significantly.
