急性脑血管病病人血浆内皮素及降钙素基因相关肽测定的临床意义

The clinical significance of detection of the plasma endothelin and calcitonin gene related peptide in patients with acute cerebrovascular disease

目的 探讨内皮素 (ET)及降钙素基因相关肽 (CGRP)水平的变化在急性脑血管病发生、发展中的意义。方法 选择发病在 4 8h内的急性脑血管病病人 ,其中脑梗死 4 0例、脑出血 2 2例 ,4 0例健康体检者作为对照组 ,采用放射免疫法测定血浆ET及CGRP。结果 急性脑梗死组ET显著高于对照组 (P <0 0 1) ,CGRP显著低于对照组 (P <0 0 5 ) ,而脑出血组ET及CGRP均显著高于对照组 (P <0 0 1,P <0 0 5 )。治疗 3周后 ,脑梗死组轻中型及恢复较好的重型病人ET降至接近正常水平 ,CGRP也升至接近正常水平 ,而重型及恢复较差的中型病人ET虽有下降 ,但仍显著高于对照组 (P <0 0 5 ) ,CGRP基本降至正常。脑出血组ET及CGRP均降至接近正常。结论 急性脑血管病发生、发展及预后与ET、CGRP的变化有关 ,监测ET及CGRP水平的变化对于急性脑血管病特别是脑梗死的病情演变及预后判断有重要意义。

Objective To explore the significance of plasma endothelin(ET) and calcitonin gene related peptide(CGRP) in the development of acute cerebrovascular disease.Methods Acute cerebrovscular disease patients including cerebral infarction(40 cases),cerebral hemorrhage(22 cases) were studied when these diseases developed within 48h and 40 healthy persons were studied as control.Their plasma ET and CGRP were dectedcted with radioimmunity assay.Results The ET level in the acute cerebral infarction was obviously higher than that in controls(P<0.01).The CGRP in the same group was clearly lower than that in controls(P<0.05).In cerebral hemorrhage cases both ET and CGRP were much higher than those in control group(P<0.01,P<0.05).After 3 weeks,the ET and CGRP levels in patients wih mild and moderate cerebral infarction and recovered servere patients returned to normal.In the serious cases and the slowly recovered cases ET was still higher than that in normal controls(P<0.05),CGRP closed to normal.Meanwhile,the ET and CGRP levels in the cerebral hemorrhage patients closed to the normal.Conclusion The development of acute cerebrovascular diseases are in contact with ET and CGRP,so monitor of ET and CGRP is very important for the patients with acute cerebrovascular disease.
