内皮素受体基因表达与肝硬化门脉高压症内脏血流动力学的关系 被引量:1

Expression of Endothelin Receptors Gene in Human Cirrhosis:Relationship with Splanchnic Hemodynamics

摘要 目的 探讨肝硬化患者肝组织中内皮素受体 (ETAR及ETBR)mRNA的表达及其与内脏血流动力学的关系。方法 肝硬化组 (n =15例 ) ,组织学检查证实为肝硬化 ;对照组为肝外伤病人 (n =10例 ) ,组织学检查证实为正常肝组织。采用原位杂交及RT PCR技术检测肝组织中ETAR及ETBRmRNA的表达 ,同时测定门静脉压力(Pp)、门脉血流量 (Qp)、平均动脉压 (MAP)和心率 (HR)。 结果 肝硬化患者肝组织中ETAR及ETBRmRNA的表达均显著高于外伤肝组织 ,ETAR及ETBRmRNA的表达与Pp呈直接相关 ,ETBRmRNA的表达与Qp呈直接相关。结论 ETAR及ETBRmRNA的表达与门脉高压症的严重程度直接相关 。 Objective To assess the correlation likely exists in increased portal pressure (Pp),portal blood flow quantity(Qp)and ET A and ET B receptor mRNA expression in human cirrhosis.Methods The study included 25 subjects(17 men and 8 women),diagnosed as cirrhosis( n =15) and as traumatic liver injury( n =10).In situ hybridization and reverse transcription polymerase chain reactions(RT PCR) were performed to determine the expression of ET A and ET B receptor mRNA in liver tissues from both traumatic and cirrhotic patients whose hepatic hemodynamic values were measured.Results Both expressions of the two transcripts were significantly higher in cirrhotic patients than those in traumatic subjects.There was a direct relationship between mRNA levels of ET A and ET B receptor and the levels of Pp and Qp in cirrhotic patients.Conclusion Liver paracrine endothelin system may be overactivated in human cirrhosis accompanied with increased expression of ET A and ET B receptor mRNA.It also shows that a direct relationship exists between endothelin receptor mRNA levels and the degree of portal hypertension in these patients.ET A and ET B receptor may play an important role in the pathogenesis and maintenance of splanchnic hyperdynamics.
