摘要
目的探讨人类乳腺浸润性导管癌中CD2 0、CD45RO、CD68的表达与乳腺癌的生长、浸润及转移的关系 ,为乳腺癌的预防和免疫治疗服务。方法采用DakoEnvision法对 30例乳腺癌中的CD2 0、CD45RO、CD68进行免疫组化染色。结果 30例乳腺浸润性导管癌 ,其中有淋巴结癌转移的 1 5例 ,CD2 0阳性 8例 ,弱阳性 3例 ,阴性 4例 ;CD45RO阳性 0例 ,弱阳性 0例 ,阴性 1 5例 ;CD68阳性 9例 ,弱阳性 5例 ,阴性 1例。无淋巴结癌转移的 1 5例 ,CD2 0阳性 4例 ,弱阳性 1例 ,阴性 1 0例 ;CD45RO阳性 4例 ,弱阳性 1例 ,阴性 1 0例 ;CD68阳性 7例 ,弱阳性 3例 ,阴性 5例。即有淋巴结癌转移组CD45RO表达低 ,CD2 0表达高 ;无淋巴结癌转移组CD45RO表达高 ,CD2 0表达低。X2 检验 ,CD2 0 :0 .0 2 5 <P <0 .0 5 ,两组之间存在差异 ;CD68:0 .0 5 <P <0 .1 ,两组之间无明显差异。四格表确切概率法检验 ,CD45RO :0 .0 2 1 <P<0 .0 5 ,两组之间存在差异。经数理统计分析 ,每组内各指标之间无显著性差异 (P >0 .0 5 )。结论乳腺浸润性导管癌中CD45RO低表达者往往有转移 。
Objective To study the relationship between expressions of CD20, CD45RO and CD68 and the growth, infiltration and metastasis in human infiltrating duct cancer of mammary glands to serve the prevention and immunotherapy of mastadenoma.Methods The method of Dako Envision was adopted for the immunohistochemical staining of CD20, CD45RO and CD68 in 30 cases of mastadenoma. Results Of the 30 cases of infiltrating duct cancer of mammary glands, metastasis of lymphadenoma occurred in 15 cases, with eight cases being CD20 positive, three being hypopositive, four being negative; no case was CD45RO positive and CD45RO hypopositive, and 15 cases were CD45RO negative; CD68 was positive in 9 cases, hypopositive in 5 cases and negative in one case. Of the 15 cases free from metastasis of lymphadenoma, CD20 was positive in 4 cases, hypopositive in one case and negative in 10 cases; CD45RO was positive in 4 cases, hypopositive in one case, and negative in 10 cases; CD68 was positive in 7 cases, hypopositive in 3 cases and negative in 5 cases. To sum up, expression of CD45RO was low while expression of CD20 was high in the group of metastasis of lymphadenoma; expression of CD45RO was high while expression of CD20 was low in the group without metastasis of lymphadenoma. Chi-square test indicated CD20: 0.025<P<0.05, with difference in the two groups, while CD68: 0.05<P<0.1, without marked difference in the two group. Test of exact probabilities in 2×2 table revealed CD45RO: 0.021<P<0.05, with difference in the two groups. Mathematical and physical statistical analysis showed no marked difference existed in the indexes of the same group(P>0.05). Conclusion Metastasis tended to occur in cases of infiltrating duct cancer of mammary glands with low CD45RO expression. This reveals that antitumour immunity is mainly cell immunity in the organism in the case of mastadenoma.
