摘要
目的探讨TET基因家族成员(TET2)突变、FLT3基因内部串联重复(FLT3?ITD)突变急性髓细胞白血病(AML)患者临床和实验室指标差异。方法收集2016年1月至2018年5月间广州中医药大学第一附属医院就诊的60例AML患者。其中TET2突变(+)12例,FLT3?ITD突变(+)8例。收集患者年龄、性别、初诊外周血白细胞、血红蛋白、血小板、外周血原始细胞比例、骨髓细胞免疫表型及核型等临床资料。结果 TET2突变AML患者在性别、年龄、血小板数、外周血白细胞、原始细胞百分数、骨髓细胞免疫表型表达率及细胞遗传学危险分层方面差异无统计学意义(P>0.05),FLT3?ITD突变外周血白细胞数增高,外周血原始细胞百分数比例增高,细胞遗传学危险分层高危比例增高,差异有统计学意义(P<0.05)。结论 FLT3?ITD突变可用于评估AML患者预后。
Objective To explore the difference between clinical and laboratory indexes in patients with ten?eleven?translocation protein 2(TET2)mutation and FLT3 internal tandem duplication(FLT3?ITD)mutation acute myeloid leukemia(AML).Methods 60 cases of AML from January 2016 to May 2018 who were treated in the First Affiliated Hospital of Guangzhou University of Chinese Medicine were selected for the study.Among them,there were 12 cases of TET2 mutation(+)and 8 cases of FLT3?ITD mutation(+).The clinical data of age,sex,white blood cells,hemoglobin,platelets,peripheral blood primordial cells,bone marrow cell immunophenotype and karyotype were collected.Results There was no significant difference in sex,age,number of platelets,peripheral blood leukocyte,percentage of primitive cells,expression rate of immunophenotype in bone marrow,and cytogenetic risk stratification in patients with TET2 mutation of AML(P>0.05).The number of leukocytes in peripheral blood was high,the percentage of peripheral blood primordial cells increased and the high risk ratio of cytogenetic risk stratification increased with FLT3?ITD mutation of AML(P<0.05).Conclusion FLT3?ITD mutations can be used to evaluate the prognosis of AML patients.
