糖尿病患者外周血单核细胞Toll样受体4基因表达及意义

The expression and significance of Toll-like receptor 4 gene in peripheral blood mononuclear cells of diabetic patients

目的探讨糖尿病患者外周血单核细胞Toll样受体4(TLR4)基因表达及意义。方法选取本院2017年1月至2018年12月收治的2型糖尿病患者164例(观察组),其中有糖尿病并发症患者32例,同时选取健康体检者170例作为对照组,采用RT-PCR检测外周血单核细胞TLR4 mRNA表达,ELISA法检测血清中肿瘤坏死因子α(TNF-α)、C反应蛋白(CRP)、白细胞介素-1(IL-1)和白细胞介素-10(IL-10)水平。结果观察组TLR4 mRNA相对表达量、TNF-α、CRP、IL-1和IL-10分别为(0.72±0.15)、(18.20±1.50)pg/mL、(5.03±1.27)pg/L、(14.80±4.66)pg/mL和(15.22±3.84)pg/mL,明显高于对照组的(0.33±0.10)、(7.68±1.36)pg/mL、(1.30±0.33)pg/L、(8.03±2.21)pg/mL、(9.68±3.54)pg/mL,差异有统计学意义(P<0.05)。观察组有并发症患者TLR4 mRNA相对表达量、TNF-α、CRP、IL-1和IL-10分别为(0.85±0.17)、(22.15±2.06)ng/mL、(6.76±1.10)g/L、(16.61±3.94)pg/mL和(16.70±3.10)pg/mL,明显高于无并发症患者的(0.69±0.14)、(17.24±1.94)ng/mL、(4.61±1.02)g/L、(14.36±2.72)pg/mL、(14.86±2.95)pg/mL,差异有统计学意义(P<0.05);糖尿病患者单核细胞TLR4 mRNA相对表达量与TNF-α、CRP、IL-1和IL-10呈正相关(r=0.355、0.303、0.300和0.312,P均<0.05)。结论糖尿病患者单核细胞TLR4 mRNA表达明显上调,与患者血清炎症因子有一定关系。

Objective To investigate the expression and significance of Toll-like receptor 4(TLR4)gene in peripheral blood mononuclear cells of diabetic patients.Methods 164 patients with type 2 diabetes mellitus(observation group)in our hospital from January 2017 to December 2018 were selected,including 32 patients with complications of diabetes mellitus,and 170 healthy persons were selected as control group.the expression of TLR4 in peripheral blood mononuclear cells was detected by RT-PCR,and tumor necrosis factor-α(TNF-α),C-reactive protein(CRP),interleukin-1(IL-1)and IL-10 were detected by ELISA.Results The relative expression of TLR4,TNF-α,CRP,IL-1 and IL-10 in the observation group were(0.72±0.15),(18.20±1.50)pg/mL,(5.03±1.27)pg/L,(14.80±4.66)pg/mL and(15.22±3.84)pg/mL,respectively,which were significantly higher than those in the control group(P<0.05).In the observation group,the relative expression of TLR4,TNF-α,CRP,IL-1 and IL-10 in patients with complications were(0.85±0.17),(22.15±2.06)ng/mL,(6.76±1.10)g/L,(16.61±3.94)pg/mL and(16.70±3.10)pg/mL,which were significantly higher than those in patients without complications(P<0.05).The relative expression of TLR4 in monocytes of diabetic patients was positively correlated with TNF-α,CRP,IL-1 and IL-10(r=0.355,0.303,0.300 and 0.312,P<0.05).Conclusion The expression of TLR4 in monocytes of diabetic patients was significantly up-regulated,which was related to the serum inflammatory factors.
