miR-449a、miR-26a和CXCL5在溃疡性结肠炎中的表达水平及其预后研究 被引量:8

Expressions levels of miR-449a, miR-26a and CXCL5 in ulcerative colitis and their prognosis value

摘要 目的探讨miR-449a、miR-26a、CXC趋化因子配体5(CXCL5)在溃疡性结肠炎组织中的表达水平及其预后研究。方法选取2017年5月至2018年4月本院进行治疗的94例溃疡性结肠炎患者,以及同期行单发息肉切除的72例肠粘膜组织正常者进行研究,所有受试者均行结肠镜检查采集结肠粘膜组织,实时定量逆转录聚合酶链式反应(qRT-PCR)检测组织中miR-449a、miR-26a表达水平,免疫组化法检测组织中CXCL5阳性表达情况,分析miR-449a、miR-26a和CXCL5与溃疡性结肠炎患者病理参数的关系;采用COX回归分析、KM曲线分析miR-449a、miR-26a和CXCL5与溃疡性结肠炎患者预后的关系。结果与正常组比较,溃疡性结肠炎组中miR-449a、miR-26a表达水平下降,CXCL5阳性表达率升高,差异有统计学意义(P<0.05)。miR-449a、miR-26a和CXCL5表达与患者疾病活动程度、内镜分级、组织学分级相关,差异有统计学意义(P<0.05)。COX单因素分析显示,疾病活动程度、内镜分级、组织学分级、miR-449a、miR-26a、CXCL-5与患者预后有关,差异有统计学意义(P<0.05);多因素显示,疾病活动程度、内镜分级、miR-449a、miR-26a和CXCL5为影响患者预后的独立危险因素,差异有统计学意义(P<0.05)。miR-449a、miR-26a低表达组和CXCL5高表达组黏膜愈合良好率分别为49.09%、49.06%和48.28%,均低于miR-449a、miR-26a高表达组与CXCL5低表达组的71.79%、76.73%和75.00%,差异有统计学意义(P<0.05)。结论miR-449a、miR-26a在溃疡性结肠炎患者中低表达,CXCL5呈高表达,三者与患者黏膜组织预后情况有关,且为黏膜愈合不良的独立危险因素。 Objective To investigate the expression levels and their clinical significances of miR-449 a,miR-26 a and CXC chemokine ligand 5(CXCL5)in ulcerative colitis.Methods Ninety-four patients with ulcerative colitis and 72 patients with normal intestinal mucosal tissues who underwent single polypectomy were included;colonoscopy was performed to collect colonic mucosal tissues for all the subjects.The expression levels of miR-449 a and miR-26 a in the tissues were detected by using real-time quantitative reverse transcription polymerase chain reaction(qRT-PCR),the positive expression of CXCL5 was detected by immunohistochemistry,the relationships between miR-449 a,miR-26 a,CXCL5 and pathological parameters of patients with ulcerative colitis were analyzed,and the relationships between the levels of miR-449 a,miR-26 a,CXCL5 and the prognosis of patients with ulcerative colitis were analyzed by COX regression analysis and KM curve analysis.Results Compared with the normal group,the expression levels of miR-449 a and miR-26 a were decreased and the positive expression rate of CXCL5 was increased in the ulcerative colitis group(P<0.05).Correlation analysis showed that signifcant positive correlations existed between the expressions of miR-449 a,miR-26 a,CXCL5 and disease activity,endoscopic grading,histological grading(P<0.05).Univariate analysis of COX showed that correlations existed between disease activity,endoscopy grading,histological grading and prognosis.Ultivariate analysis showed that disease activity,endoscopy grading,miR-449 a,miR-26 a and CXCL5 were independent factors influencing the prognosis of patients.The mucosal healing rates in miR-449 a and miR-26 a low expression groups and CXCL5 high expression group were lower than those of miR-449 a and miR-26 a high expression groups and CXCL5 low expression group(P<0.05).Conclusion The expressions of miR-449 a and miR-26 a were decreased and CXCL5 was increased in patients with ulcerative colitis,which were all independent risk factors for poor mucosal healing and may be related to the prognosis of mucosal tissues.
