献血者Fib、FⅧ:C个体差异与冷沉淀质量相关性的探讨

Analysis of the correlation between individual differences in Fib and FⅧ:C and the quality of cryoprecipitate in blood donors

目的探讨献血者纤维蛋白原(Fib)、凝血因子Ⅷ活性(FⅧ:C)个体差异与冷沉淀质量相关性。方法统计2017年冷沉淀质量抽检结果,对不符合者进行血液采集、制备、储存、质检等过程分析,并进一步追溯检测献血者Fib、FⅧ:C。结果冷沉淀质量抽检20批次80 U,Fib、FⅧ:C符合率分别为98.75%(79/80)、90.00%(72/80)。血液采集、制备、储存、质检等过程分析符合要求。追溯1例献血者Fib含量为1.42 g/L,低于正常对照组Fib(2.57±0.47)g/L(n=21);追溯8例献血者FⅧ:C活性,40%~70%占62.5%(5/8),71%~80%占25.0%(2/8),101%~110%占12.5%(1/8)。追溯检测组FⅧ:C(69.0±18.8)(n=8)与正常对照组FⅧ:C(123.9±30.3)(n=21)比较差异有统计学意义(P<0.05)。结论献血者Fib、FⅧ:C个体差异与冷沉淀质量具有相关性,自身低Fib、FⅧ:C浓度水平影响其捐献血液所制备的冷沉淀质量。

Objective To analyze the correlation between individual differences in fibrinogen(Fib)and FⅧ:C and the quality of cryoprecipitate in blood donors.Methods Results of quality sampling of cryoprecipitate in 2017 were summarized.The processes of blood collection,preparation,storage and quality control for unqualified donors were analyzed,and the trace-checking of Fib and FⅧ:C levels of blood donors were performed.Results A total of 20 batches of 80 U cryoprecipitate were sampled.The coincidence rates of the level of Fib and FⅧ:C in blood donors were 98.75%(79/80)and 90.00%(72/80),respectively.All the processes of blood collection,preparation,storage and quality control were performed according to the standard operating procedures.The Fib concentration of one blood donor was 1.42 g/L by tracing test,which was lower than the normal control group((2.57±0.47)g/L(n=21))Meanwhile,trace-checking tests found eight blood donors to have FⅧactivity;40%~70%activity accounted for 62.5%(5/8),71%~80%activity accounted for25.0%(2/8),and 101%~110%activity accounted for 12.5%(1/8).The FⅧ:C concentration of the testing group was[(69.0±18.8)(n=8)],which was significantly lower than that of the normal control group[(123.9±30.3)(n=21)](P<0.05).Conclusion Individual differences in Fib and FⅧ:C levels in blood donors were correlated with the quality of cryoprecipitate,and lower concentrations of Fib and FⅧ:C could affect the quality of cryoprecipitate.
