摘要
目的探讨血清肿瘤标志物癌胚抗原(CEA)、鳞状细胞癌抗原(SCC-Ag)、细胞角蛋白19片段(CYFRA21-1)检测对食管鳞状细胞癌的临床诊断价值。方法选择本院2017年1月-2018年10月诊治的食管鳞状细胞癌患者60例作为观察组,选择同期在本院健康体检的60例健康者作为对照组,比较两组的血清CEA、SCC-Ag及CYFRA21-1阳性率及表达水平,比较不同临床分期食管鳞状细胞癌患者的血清CEA、SCC-Ag及CYFRA21-1表达水平,比较血清CEA、SCC-Ag及CYFRA21-1单独检测与联合检测对患者疾病及分期的诊断符合率。结果观察组血清CEA、SCC-Ag及CYFRA21-1阳性率及表达水平均显著高于对照组,差异均有统计学意义(P<0.05);食管鳞状细胞癌Ⅱ、Ⅲ期患者的血清CEA、SCC-Ag及CYFRA21-1表达水平显著高于Ⅰ期患者,差异有统计学意义(P<0.05),并且食管鳞状细胞癌Ⅲ期患者的血清CEA、SCC-Ag及CYFRA21-1表达水平显著高于Ⅱ期患者,差异有统计学意义(P<0.05);血清CEA、SCC-Ag及CYFRA21-1联合检测对疾病及分期的诊断符合率为90.0%,显著高于以上各指标单独检测的68.3%、66.7%、73.3%,差异均有统计学意义(P<0.05)。结论血清CEA、SCC-Ag及CYFRA21-1检测对食管鳞状细胞癌具有较高的诊断价值,临床分期越高各检测指标的表达水平也随之升高,联合检测能够进一步提高疾病及分期的诊断符合率。
Objective To investigate the diagnostic value of serum tumor markers carcinoembryonic antigen(CEA),squamous cell carcinoma antigen(SCC-Ag)and cytokeratin 19 fragment(CYFRA21-1)for esophageal squamous cellcarcinoma.Methods 60 patients with esophageal squamous cell carcinoma in our hospital from January 2017 to October2018 were selected as the observation group,and 60 cases of healthy subjects from the same period of healthy physicalexamination were selected as the control group.The positive rates and expression levels of serum CEA and SCC-Ag andCYFRA21-1 of two groups were compared,the serum CEA,SCC-Ag and CYFRA21-1 in patients with different clinicalstages of esophageal squamous cell carcinoma were compared,and the diagnostic compliance rate of disease and stagingwith individual detection and combined detection were compared.Results The positive rates and expression levels of serumCEA,SCC-Ag and CYFRA21-1 of observation group were significantly higher than those of control group(P<0.05);theexpression levels of serum CEA,SCC-Ag and CYFRA21-1 in patients with stageⅡandⅢesophageal squamous cellcarcinoma were significantly higher than those in stage I patients(P<0.05),and the levels of serum CEA,SCC-Ag andCYFRA21-1 in patients with stageⅢesophageal squamous cell carcinoma were significantly higher than those in stageⅡpatients(P<0.05).The diagnostic compliance rate of disease and staging of serum CEA,SCC-Ag and CYFRA21-1 levelscombined detection was 90.0%;it was significantly higher than 68.3%,66.7%and 73.3%of the above indicators inseparate detection(P<0.05).Conclusion Serum CEA,SCC-Ag and CYFRA21-1 levels had high diagnostic value foresophageal squamous cell carcinoma,and the higher the clinical stage,the higher were the expression level of each testindex;the combined detection could further improve the diagnostic compliance rate of disease and staging.
