体外循环心内直视手术时GIK溶液中胰岛素及氯化钾用量的初步探讨

Study of the Dosage of Insulin and KCL in GIK Solution Used in Open Heart Surgery

比较体外循环心内直视手术中20例用平衡盐液与18例用 GIK 溶液的血糖、血钾变化。血糖水平两组间无明显差异(P>0.05),均于麻醉后即上升,体外循环结束及手术结束达最高值。血钾均呈逐渐下降趋势。换瓣病例补钾及尿量较先心病者为多,可能与转机后组织灌流改善有关。应用上述 GIK中的胰岛素剂量,尚不足以控制血糖至正常范围,是否需适当提高胰岛素和氯化钾用量,有待进一步探讨。

The balanced salt solution was used in 20 cases.In other 18 cases with GIK solution.The serum glucose level betweenthese two groups had no significant variation.But they rose gradually and reached the peak level at the end of the extracorpore-al circulation and the operation.Although the levels of serum potassium were in the normal ranges,but it gradually dropped tothe lowest level at the end of the operation.Due to the fact that the level of serum glucose remains same in both groups,so it needs to study further the dosages of in-sulin and KCI appropriately to maintain the homeostasis of the body.
